Objectives?1?To describe the current status of exercise among kidney transplant recipients.?2?To construct exercise model based on the health action process approach model,to verify applicability of the model and to explore the influencing factors of exercise behavior and exercise intention.MethodsFrom July in 2018 to October in 2018,a convenience sample of 320 kidney transplant recipients was recruited when they visited the Urology clinic at Beijing Chao-Yang Hospital,Capital Medical University.They were investigated for the first time by the general questionnaire,Pre-actional Self-efficacy Scale,Outcome Expectations Scale,Risk Perception Scale,Exercise Intention Scale,Exercise Social Support Scale,the Tampa Scale for Kinesiophobia and the Exercise Stage scale.After 3 months,the same sample was investigated for the second time by Exercise Planning Scale,Coping Self-efficacy Scale,Recovery Self-efficacy Scale and the Leisure-time Physical Activity part of the International Physical Activity Questionnaire.IBM SPSS 22.0 software was used for the statistical analysis.Amos 21.0 was used to explore the impact of latent variables on exercise behavior and exercise intention.Results?1?252 kidney transplant recipients were included in the end.There were 163 males and 89 females.Mean age was 43.18 years old?SD 11.27;range 1872years?.The median duration after kidney transplantation was 5 years?range 0.5 to 24.33 years?.?2?The median exercise of kidney transplant recipients was 924.00 MET-min/w(P25=297.00 MET-min/w,P75=1716.25 MET-min/w).177?70.24%?recipients were actors,65?25.79%?were non-intenders,and 10?3.97%?were intenders.Recipients with comorbidity had more exercise than those without?P<0.05?.?3?Results of structural equation model showed that the exercise could be explained19.8%of the variance.Exercise planning was the most important latent variable?coefficient of direct effect 0.395?;recovery self-efficacy?0.126?could influence exercise directly;exercise social support?0.009?,positive outcome expectations?0.020?,negative outcome expectations?-0.022?,pre-actional self-efficacy?0.079?,coping self-efficacy?0.228?could influence exercise indirectly,among which,exercise intention influence exercise via chain mediation of coping self-efficacy and exercise planning.Exercise intention could be accounted for 50.8%of the variance with pre-actional self-efficacy?0.529?,negative outcome expectations?-0.332?,positive outcome expectations?0.297?,and exercise social support?0.139?,and all of them influence exercise intention directly.ConclusionThere is a high level of exercise among kidney transplant recipients,but most of them conduct walking,the moderate-to-vigorous physical activity needs to be improved.The structural model based on the health action process approach was able to explain exercise and exercise intention among kidney transplant recipients.Exercise planning is the most important factor affecting exercise,followed by coping self-efficacy and recovery self-efficacy.It is suggested that the medical stuff should focus on the exercise planning and exercise self-efficacy in order to improve the kidney transplant recipients'exercise.
