Purpose:To analyze the risk factors of early-onset type 2 diabetes with higher risk of diabetic kidney disease than late-onset type 2 diabetes,to study the distribution characteristics of TCM syndrome types of early-onset type 2 diabetes with diabetic kidney disease,and to explore the correlation between TCM syndrome types and various factor.Material and method:324 patients with clearly diagnosed type 2 diabetic kidney disease who were hospitalized in the the Department of Endocrinology,Liaoning University of Traditional Chinese Medicine from May 2018 to May 2019 were selected.After screening for inclusion criteria and exclusion criteria,they were divided into early-onset group(age of initial diagnosis ≤40 years old)and late-onset group(age of initial diagnosis> 40 years old).Follow the method of random number table,60 patients were randomly selected from the study.According to the method of random number table,60 cases of medical records were randomly selected for study in each group.Clinical data were collected from both groups,including gender、age、diabetes onset age、duration、history of smoking、drinking、family history、height、weight、body mass index、waist circumference、triglyceride、total cholesterol、high density lipoprotein cholesterol、low density lipoprotein cholesterol、serum creatinine、uric acid、fasting plasma glucose、glycosylated hemoglobin A1c、fasting C peptide、1h C peptide、2h C peptide、urinaryalbumin-to-creatinine ratio、systolic blood pressure、diastolic blood pressure、high blood pressure、diabetic retinopathy、TCM syndromes.Statistical analysis of clinical data differences between the two groups of patients,logistic regression was used to analyze the risk factors of early onset type 2 diabetes with diabetic kidney disease,to observe the distribution characteristics of TCM syndrome types and the correlation between TCM syndrome types and various factors.Results: 1.There was no significant difference in age between the two groups(P>0.05);There were significant differences in disease course,BMI and WC between the two groups,and the difference was statistically significant(P < 0.05).2.There was no significant difference in sex ratio,smoking history and drinking history between the two groups(P>0.05).The family history of diabetes in the early-onset group was 56.67%,which was significantly higher than that in the late-onset group(30.00%),and the difference was statistically significant(P<0.05).3.There were significant differences in TG、TC、UA、GLU、Hb Alc、SBP、DBP between the two groups(P < 0.05),The levels of HDL-C、LDL-C、SCr、FC-P、1h C-P、2h C-P in the two groups were different,but the difference was not statistically significant(P > 0.05).The differences that combined hypertension and retinopathy were statistically significant(P < 0.05).4.Logistic regression analysis showed that the disease course,WC,Hb Alc,DBP and combined retinopathy were the risk factors of diabetic kidney disease in patients who were early-onset type 2 diabetes than who were late-onset type 2 diabetes(P<0.05).5.Distribution of TCM syndromes in early-onset group:qi and yin deficiency syndrome 21(35 percent)>yin and yang deficiency syndrome17(28.3 percent)>spleen and kidney qi deficiency syndrome1 13(21.7 percent)>yin scorching deficiency syndrome 9(15 percent),The results of one-way analysis of variance show that there were statistically significant differences in the age(F=10.778,P=0.000)and course of disease(F=7.140,P=0.000)of the four syndrome types.The course and age of yin and yang deficiency syndrome1 was higher than that of spleen and kidney qi deficiency syndrome1 and yin scorching deficiency syndrome(P<0.05).Conclusion: 1.There were significant differences in disease course、BMI、WC、SBP、DBP、TG、TC、UA、GLU、Hb A1c、combined with hypertension and retinopathy between the early-onset group and the late-onset group;2.The risk of diabetic kidney disease in early-onset type 2 diabetes is higher than the risk factors for late-onset type 2 diabetes: disease course,WC,FPG,combined retinopathy,combined with hypertension;3.Distribution of TCM syndromes in early-onset group:qi and yin deficiency syndrome>yin and yang deficiency syndrome>spleen and kidney qi deficiency syndrome1>yin scorching deficiency syndrome;The results of correlation analysis between TCM Syndrome Types and various factors show that : there were significant differences in disease course and age among the four groups,the deficiency syndrome of yin and yang deficiency syndrome has a longer course and a higher age than other syndromes. |
