Exploring The Meta-analysis Of The Relationship Between The Protective Effect Of Podocytes And The Treatment Of Diabetic Kidney Disease By Traditional Chinese Medicine

Purpose: To evaluate the correlation between traditional Chinese medicine(TCM)in the treatment of diabetic kidney disease and its podocyte protection.Materials and Methods: The computer searched CNKI,VIP,Wan Fang Data,Sino Med,and Pubmed databases,and retrospective references included in the research,collected from the establishment of the database to August 31,2019 on the relationship between traditional Chinese medicine treatment of diabetic kidney disease and podocyte protection in animals randomized controlled trial(PCT)related literature.Two researchers independently completed the literature screening,data extraction,and evaluation of the risk of bias in the included studies,and then performed a Meta analysis using Rev Man 5.3software.Because the analyzed indicators are continuous data,the standard mean difference(Standardized mean difference,SMD)and its 95% confidence interval(Confidence interval,CI)are used to express the forest plot to analyze the research results and to perform a publication bias risk assessment.Results:1.A total of 11 literatures were included in this meta-analysis,of which 10 were Chinese literatures and 1 was an English literature.There are two types of experimental animals included in the study: SD rats and Wistar rats.2.Meta analysis results show that:(1)Compared with the model group in the treatment of diabetic kidney disease rats with traditional Chinese medicine in terms of increasing the content of CD2 AP protein in kidney tissue,the combined effect amount was SMD=7.04,95%Cl[3.39,10.70],P=0.0002.Compared with the model group in up-regulating the expression of CD2 AP m RNA in the renal cortex:the combined effect amount SMD=3.31,95%Cl[1.94,4.68],P<0.00001.(2)Chinese medicine treatment of diabetic kidney disease rats compared with the model group in increasing Podocin protein in kidney tissue: combined effect amount SMD=8.27,95%Cl[4.23,12.31],P<0.0001.Compared with the model group in up-regulating the expression of Podocin m RNA in the renal cortex: the combined effect amountSMD=3.04,95%Cl[0.36,5.71],P=0.03.(3)Compared with the model group in the treatment of diabetic kidney disease rats with traditional Chinese medicine in increasing Nephrin protein content: the combined effect amount SMD=4.78,95%Cl[2.59,6.96],P<0.0001.Compared with the model group in up-regulating the expression of Nephrin m RNA in the renal cortex: the combined effect amount SMD=7.13,95%Cl[4.64,9.62],P<0.00001.(4)Compared with the model group,Chinese medicine for diabetic kidney disease rats reduced the BUN level in rat serum,and the combined effect amount was SMD=-3.20,95%Cl[-4.61,-1.80],P<0.00001;in reducing rat serum Compared with the model group in terms of Scr level,the combined effect amount SMD=-3.40,95%Cl[-4.92,-1.87],P<0.0001;compared with the model group,in terms of reducing the quantitative level of 24 hours urine protein in rats,the combined effect Effect amount SMD=-4.06,95%Cl[-6.64,-1.49],P=0.002.Conclusion: After treatment with Chinese medicine,the content of podocyte-related proteins in rats with diabetic kidney disease increased significantly,the expression of related protein genes significantly increased,the renal function of rats significantly improved,and proteinuria decreased.Therefore,the treatment of diabetic kidney disease with traditional Chinese medicine may be related to podocyte protection.
