| Purpose: To study the different stages of gastric cancer Physical and spirit damage characteristics and the rule of Chinese medicine yinpian treatment.Material and method: Retrospective study was conducted.,To collect the data of patients with gastric cancer who were first diagnosed and visited more than two times(including two times)in the Department of Oncology,Affiliated Hospital of Liaoning University of Traditional Chinese Medicine and Professor Yin Dongfeng’s outpatient clinic from March 2014 to June 2018.Several visits to patients were included in the database for the latest diagnosis and treatment since the survey,A total of 56 patients who met the inclusion criteria were collected.Microsoft Excel 2011 and SPSS20 were used to establish the database and to analyze its general data,PS score,clinical symptoms,Western medicine anti-cancer treatment and the use of Chinese herbal pieces.Results:1.Fifty-six patients met the admission criteria,including 44 in-patients,12 out-patients,34males(60.7%)and 22 females(39.3%).The age range was 34-85 years,with an average age of 64.9 years and a median age of 64 years.Among them,34 cases were in the early stage,22 cases in the middle stage,4 cases in the middle stage had only one cancer focus,and the remaining 18 cases had two or more cancer focus.The average number of visits to our hospital was 4.56(3-24)in 34 cases in the early stage,4.32(2-35)in 22 cases in the middle stage,4.95(2-35)in 44 cases in hospital and 2.67(2-6)in 12 out-patients.2.Anti-tumor treatment by western medicine: in the early and middle stage,there were 5cases(14.71%)and 7 cases(31.82%)of patients receiving anti-tumor treatment by western medicine,In the early stage,4 patients took S-1orally,1 patient took Xeloda orally;In the middle stage,4 patients took S-1orally,1 patient took Reetrexed intravenously and 2 patients took Apatinib orally.3.PS score and frequency of clinical symptoms: mean PS score of early patients was 2.088(+0.866),PS score of mid-term patients was 2.727(+0.550),there was significant differencebetween early and mid-term(p<0.05).The mean frequency of clinical symptoms was 4.206(+2.056)in early stage patients and 8.091(+2.266)in mid-stage patients.There was significant difference between early and mid-stage patients(p<0.01).4.Main clinical symptom: The total frequency of symptoms in 56 patients with gastric cancer was 326 times,and the top 11 symptoms with the total frequency were analyzed.The early symptoms with the frequency 50% mainly included fatigue,vomiting and poor tolerance,accounting for 94.12%,67.65% and 58.82%,respectively.Medium-term mainly include fatigue,vomiting,anorexia,abdominal distension,constipation,acid regurgitation,poor sleep soundly,angular,respectively accounted for 100%,100%,100%,90.91%,86.36%,86.36%,81.82%,50%,compared with the early,mid patients felt significantly upward trend,significantly increase the sad expression,in turn,has anti acid,constipation,abdominal distension,insomnia,poor appetite,nausea,vomiting,rising proportion by more than 30%.5.The use of TCM yinpian: all the 56 patients had more than 2 times of multiple visits,but this study only analyzed the cases and included 56 prescriptions of the lastest visit,involving145 traditional Chinese medicines,with a total medication frequency of 1208 times.In the early stage,there were 15.55% of TCM yinpian to treat the physique,12.82% to treat both physique and spirit,1.63% to treat the spirit;In the middle stage,there were 8.63% of TCM yinpian treat the physique,9.89%to treat both physique and spirit,81.48% to treat the spirit.Conclusion:1.The PS scores and clinical symptoms of patients with gastric cancer in the middle stage(with tumor)were worse than those in the early stage(without tumor),and the physical and mental injuries were more obvious.2.Strengthen the healthy qi(treat spirit)is based on Xiangsha Liujunzi Decoction to protect stomach qi,In the middle stage(with tumors)the spleen was strengthened and the Qi-regulating drugs were increased than in the early stage(without tumors).3.The whole course of the disease showed that the priority was to Strengthen the healthy qi/treat the spirit and to protect the stomach-qi,while the intensity of removing pathogens/physique-treating was inversely correlated with the stage of the tumors,showing the rule of paying equal attention to both strengthening the healthy qi and removing pathogens at theearly stage,and treating the spirit in the middle stage was more than treating the physique.In the course of the disease,the physique and spirit gradually transited from absolute/near absolute unification to relative/near relative unification. |
