Purpose: In this experiment,the rat sciatic nerve clamp injury model was used as a platform to acupuncture the GB30 and BL40 points for sciatic nerve function index,nerve HE staining,electron microscopy,and immunohistochemistry.Such tests confirmed the effectiveness of acupuncture treatment of sciatic nerve injury;Using nerve growth factor(NGF)and brain-derived neurotrophic factor(BDNF)as indexes,the differences in the repair effect of acupuncture at different acupoints on nerve injury were evaluated.The data of all the above indicators were combined to analyze the effect and mechanism of different acupoints on nerve repair after sciatic nerve injury.Material and method: 56 SPF-grade SD rats weighing 300±50 g were used.They were 2 to 3 months old,male and female each half,and the rats were kept in the Animal Experimental Center of Liaoning University of Traditional Chinese Medicine for a week after purchase to adapt to the experimental environment.Using a random number table method,56 SD rats were divided into blank control group,model control group,GB30 and BL40 group,GB30 group,and BL40 group,with 10 rats in each group.On the 8th day of modeling(before acupuncture treatment)and on the 22 nd day of modeling(after acupuncture treatment),each group of the clamp model(model control group,GB30 and BL40 group,GB30 group,BL40 group)Tests were performed to assess the injury and repair of the sciatic nerve in rats.In this experiment,the sciatic nerve function index(SFI)and nerve conduction velocity(MNCV)were measured in rats before and after acupuncture treatment to evaluate the recovery of neuromuscular and motor function and neurotransmission in each group before and after treatment.At the end of the last treatment(modeling day 22),rats were sacrificed by overdose anesthesia.Samples were then collected and collected,including sciatic nerve tissues up and down each 2 mm lesion.Hematoxylin-eosin(HE)staining was performed to observe the pathological changes of the sciatic nerve injury.Histological observation of the ultrastructure of the sciatic nerve at the jaws was performed by electron microscopy.The contents of NGF and BDNF in injured nerves were detected by immunohistochemical method.Finally,The results of the data were statistically analyzed.Results: 1.Sciatic nerve function index(SFI): On the 8th day of modeling(before acupuncture treatment),the SFI values in the clamp model group(model control group,GB30 and BL40 group,GB30 group,BL40 group)were significantly lower than those in the blank control group.Statistical significance(P<0.05).There was no significant difference between the model groups(P>0.05),indicating that the motor function of the model rats was reduced and the modeling was successful.On the 22 nd day of model establishment,after 14 acupuncture treatments,the index of the acupuncture treatment group(GB30 and BL40 group,GB30 group,BL40 group)did not return to the normal level of the blank control group,but compared with the model control group SFI.There was a significant increase in the values,and there was a significant difference between the two groups(P<0.01).It shows that acupuncture can promote the recovery of neuromuscular and motor function in rats with sciatic nerve injury.The SFI evaluation of the sciatic nerve was better in the GB30 group than in the BL40 group.The GB30 and BL40 group was superior to other treatment groups,with a significant difference(p<0.05)..2.Nerve electrophysiology(Motor nerve conductive velocity,MNCV)The motor nerve conduction velocity(MNCV)showed that compared with the blank group,the MNCV of the clamp model group was decreased(P<0.05)after modeling,suggesting successful modeling.After the treatment,the acupuncture treatment group was superior to the model control group(P<0.05).Among them,the GB30 group was superior to the BL40 group(P<0.05),and the GB30 and BL40 group was superior to other treatment groups(P<0.05).3.HE staining for morphological changes of sciatic nerve In the blank control group,axons,nerve sheaths,and Schwann cells were arranged neatly,with clear structures and continuous nerve fibers.In the model control group,the nerve fibers traveled disorderly,the axons disappeared,the myelin occurred Vallele degeneration,and the Schwann cells increased swelling.In the acupuncture treatment group,compared with the model control group,the nerve fiber disorder in the acupuncture treatment group was relieved,the myelin sheath only appeared dislodged and no disintegration was observed,Schwann cells increased,and new nerve fibers appeared.The distribution of sciatic nerve fibers in each group was relatively regular in acupuncture treatment.Some nerves showed new growth and some myelin shedding.Among them,the pathological improvement of GB30 and BL40 group was better than that of the other two groups.The degree of sciatic nerve repair in GB30 group was better than that of BL40 group.4.Histological observation of the ultrastructure of the sciatic nerve at the jaws by electron microscopy On the 22 nd day of modeling(after the acupuncture treatment was finished),the ultrastructure of the sciatic nerve at the jaws was observed by electron microscopy and histological observation: In the normal control group,the nerve fibers were intact,the Schwann cells were normal in morphology,the mitochondria were abundant,and the bilayer membrane structure was clear.The myelin sheaths were clearly arranged in layers without degeneration and demyelination;model control group.: Severe degeneration of nerve fibers,a large amount of vacuolar degeneration mitochondria in axon,myelin sheath release,deformation,axis mutation.The acupuncture treatment in each group was significantly better than the model control group in terms of morphology,axonal organelles,Schwann cell entrapment,and myelin sheath thickness.In the group of “GB30 and BL40 group”,the myelin structure around the axons was almost complete,the organelles were abundant,the vacuolization was reduced,the Schwann cells were normal in shape,and there were still twists and deformations in the tendons.It shows that acupuncture can promote the repair of nerve damage,and the degree of sciatic nerve repair in the " GB30 and BL40 group " is better than that in the single-point group.5.Immunohistochemical detection and Immunofluorescence detection In immunohistochemistry,the positive expression of nerve growth factor(NGF)and brain-derived neurotrophic factor(BDNF)was brownish yellow,and no staining was negative.The positive expression of NGF and BDNF in immunofluorescence was fluorescent bright green.The blank control group: normal nerves do not express or only a small amount of NGF,BDNF in the axoplasm of nerve fibers.Model control group: The positive expression of NGF and BDNF was found in the axons of regenerated and regenerated nerve fibers and in the cytoplasm of Schwann cells.The expression sites were of different size and the boundary was unclear.Compared with the blank control group,nerve fibers were disordered,Schwann cells increased,and NGF and BDNF were upregulated.The expression of neurotrophic factor(NGF,BDNF)in acupuncture group was higher than that in model control group;the positive expression of NGF and BDNF was found in the group of “ring jump acupuncture at the middle point” group.Higher than single acupoint group.Conclusion: 1.Acupuncture at GB30 and BL40 points on the sciatic nerve and branches can promote recovery of motor function in rats with sciatic nerve injury,and can increase nerve conduction velocity.2.Acupuncture at GB30 and BL40 points can improve damaged nerve fibers and promote the repair of damaged nerve structures.3.Acupuncture at GB30 and BL40 in the treatment of sciatic nerve injury can upregulate the NGF,BDNF content of the damaged nerves.
