Objectives:Multiple system atrophy is a group of sporadic,adult-onset neurodegenerative diseases.The main clinical symptoms include cerebellar ataxia,Parkinson's syndrome(insensitivity to 1-dopa drugs),vegetative nerve functional disturbance and pyramidal tract sign.Because of the diversity of its clinical manifestations and the limitation of neuropathological examination,MSA is difficult to be diagnosed,especially in the differential diagnosis of diseases such as progressive supranuclear paralysis,vascular Parkinson's syndrome,cortical basal ganglia degeneration and lewy body dementia.Studies have shown that recording and analyzing abnormal eye movements in patients with multisystem atrophy can help differentiate them from other diseases.Therefore,in this study,the clinical diagnosis of MSA patients were examined by electronystagmograms,and their eye movement characteristics were recorded and analyzed,in order to study the characteristics of electronystagmograms of different syndromes in patients with multiple system atrophy.To explore the correlation between different TCM syndromes of multiple system atrophy disease and the parameters of electronystagmogram,and to provide reference and examination basis for syndrome differentiation and classification of multiple system atrophy.Methods:Patients with multiple system atrophy in outpatient and ward of dongzhimen hospital were selected from April 2016 to December 2018.A total of 25 patients were included as subjects according to the inclusion criteria,exclusion criteria and diagnostic criteria.Take the interview questionnaire,including general information,electronystagmogram report,unified multiple system atrophy rating scale and TCM syndrome questionnaire four parts,set up the database,using spss20.0 statistical software for statistical analysis,by T test and X2 test,rank and test analysis,such as Yang deficiency syndrome,Yin deficiency syndrome in patients with multiple system atrophy of electronystagmogram and unified multiple system atrophy rating scale differences in multiple system atrophy.Results:1.General situation analysis:There were 13 males and 12 females,a total of 25 MSA patients.The average age of males was 57.15±15.23 years old,and that of females was 55.42 ±9.49 years.There was no significant difference in age between male and female groups(P=0.734>0.05).Among the 25 patients with multiple system atrophy,the young elderly(60-74 years old)are the most distributed,and the proportion of male and female patients older than 60 years old is 28%and 20%respectively.The mean course of disease was 3.642.53 years.There were 13 cases of Yang syndrome and 12 cases of Yin syndrome.There was no significant difference in gender,age and disease course between Yang deficiency syndrome and Yin deficiency syndrome in 25 patients with multiple system atrophy.2.Clinical data:2.lElectronystagmogram examinationAmong the 25 patients with multiple system atrophy,76%were abnormal in spontaneous nystagmus test,4%in gaze test,92%in scanning test,,64%abnormal in visual tracking eye movement,36%in ?/? and 64%in ?/? type.56%indicated the central abnormality,0%indicated the peripheral anomaly,44%had both.There was no significant difference in the distribution of syndromes between the results of electrotystagmogram,P=0.534>0.05.There was no significant difference in syndrome distribution among different results of spontaneous nystagmus,P>0.05.There was no significant difference in syndrome distribution among different results of gaze test,P>0,05,There was no significant difference in syndrome distribution among different results of scanning test,P>0.05.There was significant difference in syndrome distribution between different curves of visual tracking eye movement test p=0.001<0.05.2.2Unified multiple system atrophy assessment scaleThe differences in syndrome distribution between the score results of the first and second parts of the multiple system atrophy assessment scale were statistically significant,P=0.010<0.05,P=0.016<0.05,respectively.There was no significant difference in syndrome distribution between the score results of the fourth part of the multiple system atrophy assessmen scalet,P=0.376>0.05.Conclusion:The electronystagmogram can be used as a monitoring and examination method for multiple system atrophy diseases.There is a correlation between the syndrome of multiple system atrophy and the result of electronystagmogram examination,and it can be used as an auxiliary means for syndrome differentiation of multiple system atrophy in traditional Chinese medicine.This study is an initial exploratory experiment,and the sample size is too small,with certain limitations.The results of the experiment need to be carried out high-quality,multi-center,multi-level and large-sample studies to increase the credibility of the study on chestnut.
