This paper is divided into two partsThe first part is a literature review,mainly introducing the relevant pathogenesis of Chronic obstructive pulmonary disease,the six-strategies of respiratory therapy and its research progress in COPD.The second part is clinical researchObjectiveTo observe the therapeutic effect of the six-strategies of respiratory therapy on chronic obstructive pulmonary disease(COPD)with stable phase and to observe the percentage of CD3+,CD4+,CD8+,CD4+/CD8+in the Venous blood before and after treatment.To explore the role of six-strategies of respiratory therapy in promoting rehabilitation of COPD.MethodsPatients who met the stable diagnosis criteria of COPD and are graded with pulmonary function at grade I-III were randomly divided into the treatment group of respiratory therapy combined with western medicine and the control group of western medicine alone.the patients in the control group were treated with routine western medicine and health education.The patients in the treatment group were treated with a six-strategies breathing therapy on this basis.After 6 months of intervention,the CAT score,St.George’s Respiratory Questionnaire(SGRQ)score,lung function and numbers of acute exacerbations within half a year,T lymphocyte subsets were observed.Results1 Basic information:A total of 70 patients(treatment group 34,control group 36)were enrolled.During the clinical intervention,8 patients were lost due to lossing of follow-up,2 patients in the treatment group,and 6 patients in the control group.2 patients of the treatment group was excluded because of participating in other pulmonary rehabilitation tests.A total of 60 statistical records were recorded(30 in each group).Before treatment,sex,age,smoking history,numbers of acute exacerbation,severity of lung function,CAT score,SGRQ score and the percentage of CD3+,CD4+,CD8+,CD4+/CD8+ in the Venous blood were compared between the two groups,there was no statistically significant difference(P>0.05).2 Efficacy index:After 6 months,the CAT scores of the two groups were significantly decreased(P<0.05),and the SGRQ scores of the treatment group were significantly decreased(P<0.05),especially the symptoms and effects of disease on life.The percentage of CD3+,CD4+,CD8+,CD4+/CD8+ in the treatment group showed statistically significant differences compared with those before treatment(P<0.05)while those in the control group had no meaningful changes.There were statistically significant differences of CD4+%and CD4+/CD8+ between the two groups after treatment(P<0.05).After treatment,there was no significant difference of pulmonary function in the control group(P>0.05),while FEV1/FVC was significantly increased in the treatment group(P<0.05),FVC and FEV1%being on the rise,and there was no significant difference between the two groups(P>0.05).3 Correlation between lung function and peripheral blood T lymphocytes:linear correlation showed that FEV1/FVC and FEV1%were positively correlated with CD3+%,CD4+%and CD4+/CD8+,and negatively correlated with CD8+%(P<0.05).FVC was positively correlated with CD4+%(P<0.05).Multiple regression results showed that the increase of FVC was correlated with CD4+%and CD4+/CD8(P<0.05);the increase of FEV1/FVC was correlated with CD3+%(P<0.05);the increase of FEV1%was correlated with CD3+%,CD4+%and CD4+/CD8+,while the decrease was correlated with the percentage of CD8+(P<0.05).Conclusion1 Combined with the six-strategies of respiratory therapy is more effective than the simple western medicine in improving the symptoms and improving the quality of life.2 Six-strategies of respiratory therapy can treat the stable phase of COPD by cellular immunity of regulating helper T cells and reducing cytotoxic T cells. |
