Objective:To discuss the clinical effect of Niao-Du-Kang mixture in the treatment of patients with acute renal injury(AKI),and to study the protective effect and protective mechanism of Niao-Du-Kang mixture on renal ischemia-reperfusion by animal experiments.On the basis of clinical and experimental data,the protective role of Niao-Du-Kang mixture in the treatment of acute renal injury(AKI)was clarified,and the mechanism of Niao-Du-Kang mixture on acute renal injury was explored through related animal experiments.Methods:In this clinical trial,30 patients with acute renal injury were selected and divided into treatment group and control group according to prospective clinical study method,and 30 patients with acute renal injury were divided into treatment group and control group,and 30 patients with AKI were divided into treatment group and control group.15 patients in the control group were given routine western medicine and 15 patients in the treatment group were given not only the corresponding western medicine treatment,but also the Niao-Du-Kang mixture for 2 weeks.In the animal experiment,60 SD healthy male rats were randomly divided into the following five groups,They were sham-operation group(group S),kidney ischemia-reperfusion group(group I/ R),Niao-Du-Kang mixture(low dose group)(group 3),Niao-Du-Kang mixture group(group 4)and Niao-Du-Kang mixture group(group 5).60 rats were anesthetized by intraperitoneal injection of 3% chloral hydrate.The median abdomen was used as the incision.After incision,the abdominal muscles were obtuse into the kidney layer by layer,and then the abdominal muscles were obtuse and dissociated.The renal artery was separated layer by layer,and thebilateral renal pedicle was fixed with non-injuried vessel clamp for 45 minutes,and the blood flow of the two kidneys was recovered.The gradual restoration of the renal blood flow from dark purple to normal color indicated that the renal ischemia reperfusion was successful.The renal ischemia-reperfusion model was established.After 24 hours of renal ischemia-reperfusion,the blood of abdominal aorta was taken to detect the serum levels of Scr-bun and IL-6/ TNF-? IFN-?,and the kidneys were further stripped from the blood samples of each group.At the same time,the pathological structure of the kidney was observed by HE staining under microscope.Results:The patients in the treatment group and the control group were treated accordingly,and it was found that the decrease of bun SCR in the treatment group was significantly lower than that in the control group.After modeling successfully,the indexes of Scr-bun in rats were increased at different levels.After the rats were treated with Niao-Du-Kang mixture by stomach one week before operation,compared with the model group,Niao-Du-Kang mixture was lower than that of the model group,and the down-regulation effect of serum Scr-bun in the middle and high dose groups was more significant than that in the model group.At the same time,it was found that the renal tubular structure was abnormal and pathological changes were obvious in the renal ischemia-reperfusion group,and there were obvious edema and degeneration of renal tubular epithelial cells,secondary structural disorder,abnormal lumen structure and other pathological changes,which suggested that the model was successful.The disease location of ischemia-reperfusion injury was in the kidney of rats.The pathological changes of the kidney increased gradually with the decrease of blood oxygen content,while Niao-Du-Kang mixture was low,and the middle and high dose groups had edema and degeneration of renal tubule cells.The pathological changes such as abnormal lumen structure weresignificantly reduced compared with IRI group.ConclusionThe results showed that the total effective rate of the treatment group was better than that of the control group,suggesting that Niao-Du-Kang mixture had a good protective effect on the renal function of the patients with acute renal injury.Niao-Du-Kang mixture can improve the function and immune function of IRI rats,alleviate the pathological changes of kidney and improve the inflammatory reaction of AKI rats by down-regulating the level of serum Scr-bun and IL-6,TNF-?,IFN-? in IRI rats.It can reduce the pathological changes of kidney and protect the pathological structure of kidney in rats.
