Object:To investigate the clinical effects of reconstruction of mandibular defects using individualized implant which produced by three dimensional printing technology.Method: Selecting 5 patients with mandible tumor of PLA 401 Hospital of oral and Maxillofacial Surgery Department,including 3 cases of benign tumor,2 cases of malignant tumor.All of them were left different degrees of mandibular defects after bamputating diseased tissue,defects were not crossing the midline,Among them,1 cases of patients with lesions involving the ipsilateral condyle.The data of three-dimensional mandibular model can be obtained by CT scan before operation in all patients.Among 5 cases,4 cases apply of three dimensional printing technology making resin model of mandible and to prebend the titanium reconstruction plate according to the entity model before the operation;1 case put the well-designed implant data into the three dimensional printing machine and the individualized implant with condyle was manufactured.The surgery was operating according to preoperative design.Iilia bone graft at the same time of the benign tumor.The patients were followed up timing.Selecting the operation time,bleeding volume,mandibular position,the asymmetry of the mandible,open-mouth degree,overbite,overjet to carry on the correlation research.All of the date was analyzed by SPSS 17.0 software package.Result: In this study,5 patients were operated successfully,and the wounds healiing of 4 patients were first intention,operative incision infection occurred in 1 cases.2 patients of Iliac flap survived well.And no related complications occurred.The operation time and the amount of bleeding were significantly shorter than the traditional operation method.There was no significant change in the position of the mandible before and after operation.The rate of non symmetry of the mandibewas stable in the normal range,and the symmetry of the face was good.All the patients of open-mouth degree,overbite and overjet were satisfied after operation.The satisfaction of patients on the treatment process and the effect of surgery can up to 80%.Conclusion:Using three dimensional printing technology to repair mandibular defects,not only shorten the operation time,reduce the surgical trauma,but also improve the accuracy of mandibular reconstruction.At the same time,the function of mandible was recovered well.It can achieve the effect of beauty and function.It provides a fast and effective method for the simulated reconstruction of mandible.
