Objective: Roux-en-Y digestive tract reconstruction is a common operation in radical gastrectomy for gastric cancer.This paper retrospectively analyzed the relationship between the Roux-en-Y digestive tract reconstruction and the formation of gallstones in patients with gastric cancer,and preliminarily discussed the influence of Roux-en-Y digestive tract reconstruction on the formation of gallstones,so as to guide the clinical development of relevant strategies and preventive measures.Methods: Retrospective analysis of 314 cases of gastric cancer patients with Roux-en-Y of gallstone disease happens after the digestive tract reconstruction,research Roux-en-Y digestive tract relationship with gallbladder stone formation,using the single factor and multiple factors logistic regression analysis to explore the gallbladder stone formation factors,using survival analysis to explore the gallbladder stone formation time,the influence of Roux-en-Y digestive tract reconstruction on gallstone formation was analyzed comprehensively.Results:1.In this study,314 patients with gastric cancer were followed up for21.9±3.4months after Roux-en-Y digestive tract reconstruction.,survival analysis showed that the incidence of cholecystolithiasis after Roux-en-Y digestive tract reconstruction was 14%,higher than the general population,the median time for cholecystolithiasis after surgery was 8.9 months.2.The diameters of gallstones in the experimental group were all less than 20 mm,79.55% of which were silt-like stones or stones with diameters less than 10 mm,and no stones with diameters more than 20 mm were found.3.Univariate logistic regression analysis showed that age(P=0.004),history of type 2 diabetes(P=0.004),postoperative weight loss(P<0.001),and preoperative statin use(P=0.049)were associated with postoperative gallstones.4.Multivariate logistic regression analysis showed that age(P=0.028),total gastrectomy(P=0.032),and percentage decrease in total weight(P<0.001)were independent risk factors for postoperative gallstones.Conclusions:1.The incidence of gallstones after Roux-en-Y digestive tract reconstruction of gastric cancer is higher than that of the general population.The reason is that gallbladder movement disorder and gallbladder metabolism disorder are caused by surgery.2.The occurrence of gallstones after Roux-en-Y digestive tractreconstruction of gastric cancer is related to the patient's age,history of diabetes,degree of postoperative overall weight reduction,total gastrectomy and preoperative administration of lipid-lowering drugs,among which age,total gastrectomy and degree of postoperative overall weight reduction are independent risk factors.3.For patients with the above risks,regular postoperative review,lifestyle adjustment,and oral medication should be used to reduce the occurrence of gallbladder stones,and psychological counseling should be conducted;For patients with gallbladder stones found before surgery,surgical indications should be strictly followed.
