Objective: By analyzing the OPRM1 A118 G gene polymorphism in Zhuang nationality female patients in western Guangxi,the influence of OPRM1 A118 G gene polymorphism on the analgesic effect of sufentanil after hysterectomy was discussed,providing a basis for Zhuang nationality female patients in western Guangxi to choose individualized sufentanil postoperative analgesic program.Methods: A total of 90 patients with selective laparoscopic hysterectomy under general anesthesia were selected,aged 40-55 years,American Society of Anesthesiologists(ASA)physical status of ?-?.These patients were divided into two groups based on nationality: 45 in the Han group and 45 in the Zhuang group.All patients entered the study with the approval of the hospital ethics committee and signed the informed consent.All patients were given general anesthesia.After anesthesia,5ml of peripheral venous blood was collected and placed into the sodium citrate anticoagulant tube and stored in the-80 ? refrigerator,and the genotyping of OPRM1 was detected by polymerase chain reaction-restriction fragment length polymorphism.All anesthetic drugs were discontinued at the end of the operation,and sufentanil was given intravenously at 0.2 ?g/kg for prophylactic analgesia.Postoperative analgesia was treated with sufentanil analgesic pump.Visual Analogue Score(VAS),Ramsay sedation Score,sufentanil consumption and adverse reactions were observed and recorded at 6h(T1),24h(T2) and 48h(T3) after surgery.Results:(1)There were no statistically significant differences in age,height,weight,operation time and anesthesia time between the Zhuang group and the Han group(P>0.05).(2)In the Zhuang group,there were 12 cases of AA type,21 cases of AG type,9 cases of GG type,the allele frequency of A was 53.6%,and that of G was 46.4%.In the Han group,27 cases of AA type,12 cases of AG type,3 cases of GG type.The allele frequency of A and G was78.6% and 21.4%.The G allele frequency of Zhuang group was significantly higher than that of Han group,and the difference was statistically significant(P<0.05).(3)The average sufentanil consumption was 106.38±9.60 ?g in the Zhuang group and 102.00±9.11 ?g in the Han group,with statistically significant difference between the two groups(P<0.05).The results of the comparison in each group were the same.Compared with AA type and AG type,the mean consumption of GG type sufentanil increased,and the difference was statistically significant(P<0.05).Compared with AA type,AG type sufentanil consumption increased on average,with statistically significant difference(P<0.05).(4)VAS score,Ramsay score and incidence of adverse reactions had not significant difference between the Han group and the Zhuang group(P>0.05).Conclusion:(1)In the female population in west Guangxi region of China,OPRM1A118 G mutation frequency of Zhuang population was significantly higher than that in the Han population,and the mutation frequency was 46.4%.(2)OPRM1 A118 G gene polymorphism increased the average consumption of sufentanil in Zhuang nationality female patients within 48 h after surgery,but did not increase the incidence of adverse reactions.(3)Patients with OPRM1 A118 G gene mutation(GG type and AG type)should increase the dosage of sufentanil after surgery.Preoperative detection of patients' genotype can help patients to comfort,precision and individualized treatment.
