Objective:Comparing TNFRSF6B with the Acute Physiology and Chronic Health Evaluation??APACHE??scores and another plasma markers to explore the association of TNFRSF6B and the prognosis of ARDS.Methods:58 patients with the 2012 Berlin definition of Acute Respiratory Distress Syndrome were studied.Based on the Pa O2/Fi O2of those patients,they were divided into mild?n=16?,moderate?n=21?,and severe?n=21?groups.Meanwhile,they were also divided into survival group?n=29?and death group?n=29?based on their own 28-day clinical outcomes.All patients'clinical data,APACHE II scores and SOFA scores were recorded.Serum levels of TNFRSF6B and PCT were measured on the first 24-hours,and correlated with the survival status on Day 28 after the onset of ARDS.Results:?1?The q RT-PCR results showed that TNFRSF6B was lowly expressed in healthy people and high in the ARDS patients?P<0.05?.The relative expression of TNFRSF6B m RNA gradually increases as the severity of ARDS increases?P<0.05?.The peripheral blood TNFRSF6B m RNA expression is higher in dead patients than in surviving patients?P<0.05?.?2?The results of ELISA showed that the serum TNFRSF6B protein concentration is higher in ARDS patients than in healthy people?P<0.05?.Serum TNFRSF6B protein concentration gradually increases with the worsening of the disease?P<0.05?.The serum TNFRSF6B concentration was higher in the dead patients than in the surviving patients?P<0.05?.?3?APACHE?score was positively correlated with patient's condition?P<0.05?.Dead patients'APACHE?scores are higher than surviving patients'?P<0.05?.SOFA score was also positively correlated with patient's condition?P<0.05?.But the difference in SOFA score between death patients and survival patients has not statistically significant?P>0.05?.?4?The Pearson correlation analysis showed that the serum TNFRSF6B protein concentration was positively correlated with ARDS patients'APACHE?score?P<0.05?.Logistic regression analysis showed that high serum TNFRSF6B levels and high APACHE?scores were independent risk factors affecting the clinical outcome of ARDS.In the drawn ROC curve,AUC?TNFRSF6B?=0.719,P<0.05,95%CI=0.587-0.85,AUC?APACHE??=0.69,P<0.05,95%CI=0.550-0.829.Conclusion:?1?The expression of TNFRSF6B in peripheral blood of ARDS patients is increased,and the expression level of TNFRSF6B is positively correlated with the severity of ARDS.?2?High serum TNFRSF6B levels prompt ARDS patients'poor prognosis.
