Observation On The Curative Effect Of Combined Anti-osteoporosis Combined With Percutaneous Vertebroplasty On Osteoporotic Vertebral Compression Fractures In The Elderly

Objective This study investigated the clinical efficacy of percutaneous vertebroplasty(PVP)combined with antiosteoporosis(OP)in the treatment of senile osteoporotic vertebral compression fracture(OVCF).Methods In this study,90 elderly patients from the Affiliated Hospital of Guilin Medical College were selected from the OVCF patients admitted between May 2018 and may 2019.According to the method of random number table,they were divided into two groups,the observation group and the control group,each with 45 cases.The control group was treated with PVP operation,after which calcium and vitamin D were given,and postoperative exercise was guided.The observation group combined with anti osteoporosis on the basis of the control group Pine for comprehensive treatment.PVP was treated with PMMA through unilateral pedicle approach.According to the specific situation of patients,four kinds of anti OP drugs were given in the anti OP treatment plan,and the patients were instructed to adjust their life style.The operation indexes such as operation time,intraoperative hemorrhage,bone cement injection,hospitalization time and cost were compared.The vertebral correction,vertebral pain,vertebral function improvement and bone density improvement before and after treatment for 7 days,3 months and 1 year were compared between the two groups.The changes of bone turnover indexes before and after treatment were compared between the two groups The postoperative complications of the two groups were compared.Result The results showed that there was no significant difference between the two groups in the average time of operation,the average amount of bleeding during operation and the average amount of bone cement injection(P > 0.05).Compared with the control group,after treatment,the length of stay in the observation group was significantly reduced,and the cost of stay in the observation group was significantly reduced(P < 0.05);there was no significant difference between the two groups at the level of 3 months and 1 year after operation and 7 days after operation(P > 0.05);There was no significant difference in the anterior edge height and the central column height of the vertebrae between the two groups(P < 0.05).There was no significant difference in the anterior edge height and the central column height of the vertebrae between the two groups(P < 0.05).There was no significant difference in the posterior edge height of the vertebrae and the Cobb angle of the injured vertebrae(P > 0.05).There was no significant difference in the VAS score,ODI score and BMD between the two groups(P > 0.05))Compared with the control group,1 In,the observation group had lower vas and ODI scores(P < 0.05),and higher BMD(P < 0.05).There were significant differences in VAS,ODI scores and BMD values between the two groups at all time points before and after treatment(P < 0.05);in the control group,the vas and ODI scores were significantly higher at one year than at three months after operation,and BMD had no significant change(P > 0.05);before and after treatment There was no significant difference in the level of Ca and P between the two groups(P > 0.05);before treatment,there was no significant difference in ALP,BGP,PINP and CTX between the two groups(P > 0.05);compared with the control group,after treatment,the level of four indicators in the observation group was significantly higher(P < 0.05);the leakage of bone cement,the uneven distribution of bone cement and the incidence of deep vein thrombosis were not found in the two groups The incidence of back pain and re fracture in the observation group was significantly lower than that in the control group(P < 0.05).Conclusion The combination of PVP and anti osteoporosis comprehensive treatment can achieve better clinical effect in the treatment of osteoporotic vertebral compression fracture in the elderly,the increase of bone mass and bone metabolism is more significant,the correction of kyphosis,the increase of injured vertebral height,and the long-term effect is more stable,and the incidence of re fracture is low,which is worthy of clinical application.
