Objective: Bladder cancer is a malignant tumor of the bladder that occurs in the urothelium and is one of the most common malignant tumors of the urogenital system.With the deterioration of the ecological environment,changes in diet structure,smoking and other factors,the incidence of bladder cancer is still on the rise worldwide.According to histopathological classification,bladder urothelial carcinoma,squamous cell carcinoma,adenocarcinoma,bladder urachal duct carcinoma,bladder cancer of mesenchymal tissue origin,etc.,and urothelial carcinoma is the most common,accounting for more than 90%,followed by squamous cell carcinoma,adenocarcinoma,etc.The subtypes of bladder cancer molecules can be divided into basal type,lumen type and wild p53-like type according to different pathological types of markers,and studies have shown that the survival prognosis of wild-type p53-like type in bladder tumors is the best,while that of basal type is the worst.Bladder Cancer can be divided into the Muscle invasive Bladder Cancer and No-Muscle invasive Bladder Cancer,including MIBC accounted for about 25% to 30%,a multicenter,easy to transfer,high relapse after treatment,poor prognostic features,now in addition to the Radical Cystectomy no other clinical treatment,so to explore the molecular mechanism of the occurrence and development of MIBC change,explore new therapeutic targets and so on become a research hotspot,Mutations in oncogenes and tumor suppressor genes,tumor resistance and changes in cell signaling pathways may be related to the development of MIBC.Previous studies suggested that Tumor infiltrating lymphocytes(TILs)have high survival prognosis in breast cancer,melanoma and other tumors.However,the expression prifile of TILs and its role in MIBC remain unclear.This study aimed to analyse expression of TILs and its clinical significance in patients with MIBC.Methods : Bioinformatics is to use the computer as a basic tool to biological information storage,retrieval,and analysis of the emerging discipline,is the life sciences,natural sciences,one of the leading edge of the hot research areas focus on embodied in the whole genome and proteomics research.Through the collection and management of huge sequencing data and the analysis of numerous DNA sequences as the source,it can discover the coding regions of genetic information represented in different genome sequencing.Contained and parses the non-coding regions of genetic information,after basis for research on genetic information sorting,induction of related regulating genome genetic spectrum transcription and translation by protein spectrum information,met on evolution at the molecular level,biological metabolism,genetic development,cell differentiation,and molecular diagnosis,treatment and prognosis of disease to get further understanding,provide more basis for the prognosis of disease.In this study,data of bladder cancer patients were downloaded from TCGA database of large-scale sequencing.By the R package 、 CIBERSORT and GSEA software,the expression and prognostic of TILs in MIBC tumor tissues and non-tumor tissues were ananalyzed in publicly available databases the Cancer Genome Atlas(TCGA,the deadline was February 20,2019,version:V15.0).And through the sequencing results of MIBC tumor microenvironment,immune group analysis,combined with other clinical data analysis,different expression quantity of TILs,immune group in the prognosis of patients with MIBC effect,more in-depth understanding at the molecular level,the mechanism of action of the occurrence and development of MIBC for diagnosis,treatment,prognosis and prevention of MIBC provide a new research direction.Results:In TCGA database,T cells CD8 had the strongest positive correlation with T cells CD 4 memory activated,but are most negatively correlated with unactivated CD4 T(correlation coefficients 0.57,-0.55,respectively);Mast cells resting(P =0.002),B cells naive(P=0.006)and B cells memory(P =0.036)were highly expressed in normal tissues,macrophages M0(P =0.006),macrophage M1(P =0.009),and NK cells resting(P =0.033)mainly expressed in tumor tissues;in 404 MIBC,Highly expressed of T cells CD8(P =0.016)and T cells CD4 memory activated(P =0.023)had longer survival time than low expression,T cells CD4 resting(P =0.021)and low expression of macrophage M0(P =0.024)has a better prognosis than those with high expression;T cells CD8 expressed higher in males(P =0.007).);the higher the clinical stage of macrophage M0,the more its expression(P =0.032).MIBC according to related immune gene set is divided into high,medium and low immunity three groups,including high immunity group has higher tumor immune cell infiltration and antitumor immune effect and enrichment of the function in the signal path analysis found that the there may be some and immunoglobulin is CXCR chemokine receptor binding,protein complex,T cell chemokine regulation,antibody antigen response and other functions and pathways function,ways;The low immunity group had higher tumor purity and lower immune cell infiltration.However,there was no significant statistical significance in the prognostic survival analysis of the immune subgroup.Conclusions:The differential expression of different components of tumor infiltrating lymph node in bladder cancer reveals the clinical and biological significance of non-cancer cells constituting the tumor microenvironment.These differences may be important determinants of the prognosis and treatment response of bladder cancer patients.The high expression of TILs may serve as a potential marker for survival and prognosis of MIBC patients.In addition,the detailed analysis and immunogrouping of tumor cell immune response will help to improve the prediction of clinical therapeutic effect and identify immunotherapeutic candidate drugs... |
