| Objective: To explore the correlation between exposure to environmental chemicals and the incidence of acute lymphoblastic leukemia(c ALL)in children from the perspective of environmental factors and biological genetic factors.In order to gain differential m RNA expression profile by gene chip,we analyze the expression level of whole blood m RNA in the c ALL control group and the case group,then gain the expression of different m RNAs,and then analyze the interaction between the two in c ALL.Further provide theoretical direction for the prevention publicity of c ALL,clinical diagnosis and treatment and other medical services,and improve the health level of children in China.Methods: Through the case-control 1:1 matching study,110 cases of primary c ALL cases and 110 cases of control cases were collected.Questionnaires were used to analyze the general situation of the children,the environmental chemical exposure of the father before childbirth and the mother during pregnancy.Meanwhile,differential expression profiles of peripheral whole blood m RNA were obtained by gene chip technology,and differential metrics(MCM2,STAT4)were selected for PCR verification.Logistic regression was used to analyze the relationship between environmental chemical exposure,peripheral blood whole blood m RNA expression difference and the risk of c ALL.Logistic regression model and cross-sectional analysis were used to explore the interaction between m RNA and environmental chemical exposure.Result:(1)A total of 220 cases of initial c ALL and control group were collected by 1:1 matching of gender and age(±< 1 year old).The average age of the case group and the control group on admission to the hospital was(5.00±< 2.749)and(5.09±2.2136)years.After multivariate logistic regression analysis,the results showed that fathers had gasoline and gasoline exposure for one year before baby birth(P=0.023),decoration within 10 years(P=0.022),family tumor history(P=0.044)and children 0-3 years old insecticide exposure(P=0.014)are the risk factors for c ALL;(2)The expression level of MCM2 in peripheral whole blood of the case group was higher than that of the control group,and the expression level of STAT4 was lower than that of the control group.After adjusting for confounding factors,the differences in expression levels of MCM2(4.407,1.974-9.839)and STAT4(0.741,0.640-0.858)were still statistically significant.(3)After adjusting for confounding factors,there was no additive interaction between environmental chemical exposure factors and m RNA,and there was a positive multiplicative interaction between the high level of peripheral blood MCM2 m RNA and the 10-year house decoration history.Conclusions:(1)Father’s diesel gasoline exposure 1 year before child’s birth,decoration within 10 years,children 0-3 years old insecticide exposure,family tumor history are associated with the risk of c ALL disease;(2)Up-regulation of MCM2 expression and down-regulation of STAT4 expression in peripheral blood may increase the risk of c ALL;The addition of MCM2 m RNA and STAT4 m RNA to environmental chemical exposure factors may increase the accuracy of ALL diagnosis in children.(3)In the course of ALL pathogenesis in children,there was a positive multiplicative interaction between the high level of MCM2 m RNA in peripheral blood and the history of house decoration within 10 years.(4)Starting from the above-mentioned social and environmental risk factors,the prevention and treatment of c ALL and the publicity strategies should be improved from the three aspects of relevant medical and health management agencies,medical service units and social family groups to ensure the health of children. |
