Objective:Obesity is a chronic metabolic disease that not only directly affect the health of tissues and organs such as the brain,liver,and respiratory tract,but also the cause of diseases,for example type 2 diabetes,hypertension,hyperuricemia,which seriously affects health and life.There is neuroimaging evidence that obesity is associated with age-related brain atrophy and cognitive decline,but the changes are still definitely unclear in brain structure,brain functional activity.This study aims to explore the local functional activities and the volume of gray matter in obese patients by using the amplitude of low frequency oscillations(ALFF)and voxel based morphometry(VBM),and provide a theoretical basis for early diagnosis or prediction of outcome after treatment.Materials and methods:T1 weighted imaging BRAVO and resting-state functional magnetic resonance imaging(rs-f MRI)was performed on 38 body mass index(BMI)?30 kg / m2 and 39 healthy controls who BMI is 18.5-24.0 kg / m2,and 13 bariatric surgery patients.The resting functional magnetic resonance original data were preprocessed to calculate the ALFF values of the conventional frequency band in each brain region.The ALFF values of the conventional frequency band were compared between the two groups by using the double-sample t test.The brain regions with statistically different ALFF between the two groups were used as a mask for partial correlation analysis with BMI.And the T1 BRAVO structure image data was preprocessed by using VBM toolbox in Matlab software.The data processing process was as follows: original DICOM image was converted into NIFTI image,and row organization segmentation was performed to obtain white and gray matter images of the whole brain.Gaussian algorithm is used to check the segmented image and make it smooth,then the image of brain tissue component is modeled,and the image can be used for statistical analysis.Subsequently,two samples of the two groups were tested by t test,and the comparison results between the two groups were calibrated with GRF,with the single voxel P < 0.005,and the corrected P < 0.05.Significant differences in gray matter volume were extracted between the two groups,and partial correlation analysis was conducted with BMI.Results:1.ALFF comparison between groups: compared with the healthy control group,the ALFF values of left medial frontal gyrus,bilateral precuneus and right cingulate gyrus in the normal frequency band of the obesity group decreased;ALFF values of bilateral caudate nucleus were increased.2.VBM comparison between groups: compared with the healthy control group,the volume of gray matter in the large cerebellum of the obese group decreased,and especially in the right hippocampal gyrus,bilateral cerebellar hemispheres,left middle frontal gyrus,left precuneus and right middle inferior occipital gyrus.3.ALFF comparison before and after operation: compared with that before operation,ALFF of bilateral cerebellum,cingulate gyrus and right prefrontal lobes and cingulate gyrus increased.4.ALFF and VBM were not correlated with BMI in obese patients.Conclusion:1.Obese patients have abnormal functional activities in brain areas;2.The large cerebellar hemispheres of obese patients were damaged;3.Recovery of inhibited and controlled brain activity in obese patients after bariatric surgery.
