Purpose: Through a randomized control study,to determine the effect of Jiu Yi Dan in the treatment of Plasma Cell Mastitis after operation,and observe the effect of draining pus,eliminating slough and explore the possible mechanism.At the same time,monitoring the safety of Jiu Yi Dan for external use by testing the safety indicators.Methods: Sixty patients with Plasma Cell Mastitis after operation were divided into basic treatment group and Jiu Yi Dan group with 30 cases in each group.In the basic treatment group,Hydrotul was used to fill the postoperative sore cavity.In the Jiu Yi Dan group,Hydrotul and Jiu Yi Dan were used to fill the postoperative sore cavity.Both groups continued to use medicine until the pus decayed according to the condition of abscission of the wound cavity.The wound condition score,wound area healing rate,wound cavity volume shrinkage rate and pain score of the two groups were observed and analyzed to evaluate the curative effect of Jiu Yi Dan on draining pus and eliminating slough.The effect of Jiu Yi Dan on bacteria,macrophages and growth factors of wound was analyzed by detecting the secretion of wound.The hepatic and renal function,urinary ?2-microglobulin,urinary NAG,blood mercury,urinary mercury and secretion mercury were detected to monitor the toxicity of mercury.The collected data were analyzed by SPSS25.0 software,and the test level was ?= 0.05.When P < 0.05,the difference was statistically significant.Results:1.The score of wound condition in the first,third,fifth and seventh day after treatment in the Jiu Yi Dan group was better than that in the basic treatment group(p < 0.05);the average time of draining pus and eliminating slough in the Jiu Yi Dan group(6.67 < 0.48 days)was shorter than that in the basic treatment group(7.40 < 1.13 days)(p < 0.05).2.The positive growth rate of bacterial culture in the Jiu Yi Dan group on the 7th day after treatment(16.67%)was significantly lower than that in the basic treatment group(53.33%).3.Jiu Yi Dan had no significant effect on the contents of macrophages and growth factors(VEGF,PIGF-2,FGF,EGF)in the secretion of sore cavity on the 7th day after treatment(p > 0.05).4.Jiu Yi Dan did not aggravate the adverse reaction of pain when patients changed dressings(p > 0.05).5.Jiu Yi Dan did not affect the hepatic and renal function of patients before and after treatment(p > 0.05)when the maximum dosage of Jiu Yi Dan was not more than 1.776 g and the longest duration of medication was 7 days(p > 0.05).6.On the first day after the use of Jiu Yi Dan,blood mercury and urine mercury increased significantly,the peak value of blood mercury reached plateau phase from 3 to 5 days after using Jiu Yi Dan,and the urine mercury did not show obvious plateau phase during the treatment period.On the first day after drug withdrawal,blood mercury and urine mercury decreased significantly,and blood mercury recovered to pre-drug level on the 28 th day after drug withdrawal,and urine mercury decreased to near normal level on the 3rd month after drug withdrawal.No acute or chronic mercury poisoning was found during the study.Conclusion: Jiu Yi Dan has obvious effect of draining pus,eliminating slough and improving granulation color on the wound after operation of Plasma Cell Mastitis.Jiu Yi Dan can shorten the decomposition time of sore cavity and inhibit bacterial colonization of wound.The mercury content of Jiu Yi Dan was mainly concentrated in the secretion.The mercury content in blood and urine increased temporarily after the use of Jiu Yi Dan.After the withdrawal of Jiu Yi Dan,the mercury content in blood and urine gradually returned to normal level,and the excretion of mercury in blood was faster than that in urine.
