OBJECTIVE: Extracorporeal Shock Wave Treatment is one of the physical treatments for the treatment of osteoarthritis.Combined with the physiological and pathological characteristics of elderly patients,under the guidance of meridian theory,the effects of extracorporeal shock wave therapy were evaluated on clinical symptoms,motor function and quality of life in elderly patients with hip osteoarthritis.METHODS: Eighty elderly patients with hip osteoarthritis were enrolled in the study whose symptoms were mild to moderate.The cases came from the Department of Geriatrics and Orthopedics in Shuguang Hospital(from May 2015 to November 2018).They were divided into two groups,40 cases for each.Both groups used the extracorporeal shock wave pain treatment system for shock wave tenderness treatment(using the German STORZ MP200 ESW treatment system,energy level: 2.0 to 3.5,pulse number: 500 times per point,frequency: 15 Hz).On the basis of the control group,the treatment group increased the treatment of meridians and acupoints(energy level: 2.0-3.0 bar,pulse number: 500 times/point,frequency: 15 Hz).and acupuncture points selected the Liver Channel of Foot-Jueyin(Taichong,Zhongfeng,Ququan),the Kidney Channel of Footshaoyin(Taixi,Fuliu,Yingu).Two groups of patients were treated once a week for a total of 8 times.The WOMAC score and the health survey summary SF-36 were compared at the 0th and 2nd,4th,6th and 8th week of treatment,and the 6-minute walking distance was compared at the 0th and 8th week of treatment.Besides,the number of times that the patient used non-steroidal painkillers were recorded and the safety was evaluated.Results: 1.The two groups of patients at each time point after treatment(weeks 2,4,6 and 8)compared with before treatment,the WOMAC score and the total score of each pain scale,the total score of the stiffness scale,the daily difficulty scale are reduced to varying degrees.2.The WOMAC scale of the two groups showed that the better treatment effect of the treatment group on joint pain and stiffness was reflected in the fourth weekend,and the improvement of daily life difficulty,the treatment group was better than the control group at the 8th weekend.3.The SF-36 scale of the two groups showed that the treatment group began to affect the patient's cognition,mood and health status at the end of the sixth week,and the control group did not show significant improvement.The meridian treatment group had an intervention effect on the patient's psychological state earlier.4.The walking distance of the two groups in the six-minute treatment after 2 months of treatment was better than that before treatment,and the treatment group showed a better therapeutic advantage.5.During the treatment,except for a few cases of skin bleeding and short-term self-healing,both groups of patients did not report taking non-steroidal drugs,falls and other adverse reactions.Conclusion: The treatment of extracorporeal shock wave therapy is effective for joint pain,stiffness and improving the walking ability of patients.However,the difficulty of daily life and the improvement of patients' mental state need to be maintained for a period of time.The combination of meridian theory treatment,the joint pain,stiffness,walking exercise ability is better.In the difficulty of daily life and the improvement of the patient's psychological state,,the treatment group is more effective in the middle and late treatment period which show the long-term efficacy advantage.
