Objective The main acoustic immittance test method currently used in clinical practice in my country is still the single-frequency traditional acoustic immittance test?226 Hz for adults and children,1 k Hz for infants and young children?.The judgment of middle ear effusion caused by otitis media with effusion?OME?is mainly based on the tympanic immittance diagram.The single-component acoustic admittance test method should record the acoustic admittance with a single sounding tone frequency.It is the simplest and most commonly used method for measuring acoustic admittance.However,related studies in recent years have shown that the energy reflection in wideband tympanometry has obvious advantages over traditional single-frequency acoustic immittance in reflecting the changes in middle ear pressure caused by diseases.This study mainly explores the correlation between the absorption rate and resonance frequency of wideband tympanometry and the viscosity of the middle ear effusion,air-bone gap and its clinical significance in otitis media with effusion.Methods There are 28 patients?46ears?with OME who are admitted to our hospital from 2018-01 to 2019-03 were examined by clinical examination,electronic otoscopy,traditional tympanometry,pure tone audiometry,and the finally diagnosed in surgery.The patients were examined by pure tone audiometry,traditional tympanometry test and WBT before the surgery;obtain frequency-absorption rate?EA value?curve and middle ear resonance frequency value;calculate and calculate 0.5?2k Hz average EA value and resonance Frequency value;the external auditory canal and tympanic membrane were observed under endoscopic surgery,and the tympanic cavity was explored to observe the nature of the middle ear effusion.The correlation between absorbance,resonance frequency,middle ear effusion viscosity and air-bone gap was compared.Linear correlation analysis was used to analyze the absorption rate,resonance frequency and air-bone conduction difference,and the hierarchical correlation analysis was applied to analyze the absorption rate,resonance frequency and the viscosity of middle ear effusion.After data analysis,the corresponding correlation coefficient r will be obtained.The value of r has both positive and negative values.The positive value side represents positive correlation.On the contrary,the negative value represents negative correlation.Its range is-1?1,and when the value of r The closer the absolute value of is to 1,the closer the relationship between the two variables is.The closer to 0,the less closely the relationship is.When it is equal to 0,it indicates that there is no correlation.According to the level of?=0.01,the correlation coefficient is tested for hypothesis.When P<0.01,it is considered that there is a correlation between the two variables.Results The correlation coefficient r and P value between the absorption rate and the middle ear effusion viscosity and the air-bone gap were r1=-0.943,P1<0.001;r2=-0.960,P2<0.001;absorption rate and middle ear effusion Viscosity and air-bone gap have a negative correlation.The correlation coefficient r and P value between resonance frequency and middle ear effusion viscosity and air-bone gap were r3=-0.942,P3<0.001;r4=-0.941,P4<0.001;resonance frequency and middle ear effusion viscosity And the air-bone gap also has a negative correlation.Conclusion The absorbance,resonance frequency and the viscosity of middle ear effusion and air-bone gap are closely related,which is negatively correlated.The smaller the absorbance,the more viscous the effusion is,the larger the gas-bone conduction difference is.Similarly,the smaller the resonance frequency,the thicker the effusion,the greater the air-bone gap.The wideband tympanometry absorbance and resonance frequency can objectively judge the condition of patients with effusion-type otitis media,and can evaluate the severity of the patient's condition simply,quickly and objectively.It has guiding significance for the treatment plan and has important clinical application value.
