Objective To investigate the different expression levels of DAPK related mi R-191and PTEN related mi R-494 in fine-needle aspiration biopsy?FNAB?samples and blood of benign and malignant thyroid nodules,and to find new clinical molecular diagnostic marker and to provide new clues for the pathogenesis of thyroid cancer.Method FNAB specimens and peripheral venous blood were collected from 113 patients with thyroid nodules?48 cases of malignant and suspected malignant thyroid nodules,38 cases of nodular goiter and 27 cases of thyroid adenomas?,and calculated the sensitivity,specificity and coincidence rate of FNAB,compared with pathological results.The expression levels of mi R-191 and mi R-494 in FNAB specimens and peripheral circulation were analyzed by real-time fluorescence quantitative reverse transcription polymerase chain reaction?q RT-PCR?in 103 patients with thyroid nodules.Using SPSS18.0 statistical software to compare mi R-191,mi R-494 expression level in FNAB samples and peripheral venous blood of benign and malignant thyroid nodules;Using receiver operating characteristic curve?ROC curve?to measured sensitivity and specificity of mi R-191,mi R-494 to diagnose malignant thyroid nodules;Using Pearson correlation test to explore the correlation of mi R-191,mi R-494 expression in FNAB samples and peripheral circulation.Results?1?The sensitivity of FNAB in diagnosing thyroid cancer was 93.6%?44/47?and the specificity was 89.7%?35/39?,the diagnostic accuracy was 91.86%.?2?The relative expression levels of mi R-191 in thyroid cancer group were 0.363±0.298 and0.619±0.373 respectively,which were significantly lower than those in benign control group?P<0.05?.The relative expression levels of mi R-494 were 3.26±1.74 and 3.92±2.97respectively,which were significantly higher than the benign control group,and the difference between the groups was statistically significant?P<0.05?.?3?The area under the curve of mi R-191 and mi R-494 in FNAB and peripheral circulation was above0.7.The sensitivity and specificity of mi R-191 in FNAB and peripheral circulation were76.9%,73.5%and 61.5%,64.1%;mi R-494 were 63.6%,76.5%and 72.7%,84.6%.?4?mi R-191,mi R-494 expression level(rmi R-191=0.266,rmi R-494=0.076,P>0.05)in FNAB samples and peripheral circulation have no obvious correlation.?5?In thyroid cancer FNAB samples and peripheral circulation,the difference between the relative expression level of mi R-191,mi R-494 and the clinical characteristics of age,gender,nodule size,calcification,with or without cervical lymph node enlargement,thyroid function and thyroid antibodies with or without abnormalities were not statistically significant?P<0.05?.Conclusions?1?FNAB has a higher diagnostic value for thyroid nodules.Detection of molecular markers can be performed in FNAB specimens.?2?The relative expression of mi R-191 in thyroid cancer group is significantly lower than the benign group,while the relative expression of mi R-494 in thyroid cancer group is significantly higher than the benign group.?3?The sensitivity and specificity of mi R-191,mi R-494 is acceptable.Sensitivity and specificity of mi R-191 in FNAB is higher than in blood.Sensitivity and specificity of mi R-494 in the blood is higher than in organization.?4?Expression levels of mi R-191,mi R-494 in FNAB samples and peripheral circulation have no obvious correlation.?5?FNAB samples and peripheral blood are easily obtained,we can respectively detect the expression of mi R-191,mi R-494,to early screening and differential diagnosis for benign and malignant thyroid nodule.But FNAB specimens from thyroid nodules are more direct response to the benign and malignant nature of thyroid nodules than blood samples,and therefore more recommended for FNAB molecular marker detection.?6?The expression level of mi R-191,mi R-494 have no significant correlation with age,gender,nodule size,calcification,with or without cervical lymph node enlargement,and with or without abnormalities of thyroid function and thyroid antibodies.?7?Mi R-191 and mi R-494 can be used as molecular diagnostic markers for diagnosis of thyroid carcinoma with adjunctive FNAB.
