Analysis Of Prognostic Factors In 447 Patients With Primary Hepatocellular Carcinoma Treated Nonoperatively

Objective:Retrospective analysis of relevant medical history,laboratory examination indicators and survival time was conducted to explore the influencing factors on the prognosis of non-surgical treatment of primary liver cancer.Methods:(1)Patients who were hospitalized in the Affiliated Hospital of Youjiang Medical College for Nationalities from January 1,2016 to September 20,2018 with a definite diagnosis of PLC and no surgical treatment were selected.(2)The following data were collected: first hospital stay,gender,age,ethnicity,drinking history,smoking history,diabetes history,HBs Ag,AFP,TBIL,ALT,AST,GGT,ALB,Child-pugh class,ALBI class,portal vein tumor thrombus,treatment method,survival time.(3)For the collected data,Kaplan-Meier method was used for survival analysis,Log-rank test was used for comparison of survival time differences between groups,and Cox proportional risk model was used for multivariate analysis.Univariate analysis of meaningful variables was finally incorporated into Cox model analysis,and prognostic factors of non-surgical treatment of liver cancer were screened out.Results:(1)A total of 447 cases without missing data were retrospectively analyzed,including 385 males(86.13%)and 62 females(13.87%).The youngest age of the patients was30 years old and the oldest was 84 years old,with an average age of 57.58 ±11.59 years.The longest survival time was 60.2 months,the median survival time was 4.6 months,and the average survival time was 13.925±1.002 months.(2)The data were grouped into groups for univariate analysis,and the results showed that there were significant differences in survival time between the groups of AFP level,TBIL level,AST level,GGT level,ALB level,Child-Pugh grade,ALBI class,portal vein tumor thrombus,and treatment method.Gender,age,drinking history,smoking history,diabetes history,HBs Ag,ALT could not affect the prognosis of non-surgical treatment of liver cancer.(3)Cox proportional hazard model analysis of multivariate analysis showed that AFP level,TBIL level,AST level,Child-Pugh grade,and treatment method were independent influencing factors for the prognosis of patients with non-surgical treatment of liver cancer.Conclusion:(1)This group of patients with non-surgical treatment of liver cancer had short survival time and poor prognosis.(2)In this study,we found that the prognostic factors affecting the prognosis of patients with non-surgical treatment of HCC were: AFP level,TBIL level,AST level,GGT level,ALB level,Child-Pugh grade,ALBI class,portal vein tumor thrombus,treatment method.(3)AFP level,TBIL level,AST level,Child-Pugh grade and treatment method are independent influencing factors for the prognosis of non-surgical treatment of HCC.
