Effect Of Huoxuehuayu Decoction On NailFold Microcirculation In Patients With Systemic Sclerosis

Purpose:The purpose of this study was to investigate the effects of the treatment of Huoxuehuayu Decoction on the nailfold microcirculation and skin hardness score in patients with systemic sclerosis with Raynaud's syndrome and TCM syndrome of qi stagnation and blood stasis syndrome.Methods: From October 2016 to August 2018,the patients who were diagnosed as systemic sclerosis,and the phenomenon of Raynaud's disease appeared in the course of disease,and the syndrome differentiation of TCM was classified as qi stagnation and blood stasis syndrome in the Department of Rheumatology in Shanghai Traditional Chinese Medicine of Integrated Hospital were randomly divided into non-fumigation group,steam fumigation group and Chinese medicine fumigation group.The treatments were carried out with no fumigation hands,steam fumigation hands,and Huoxuehuayu Decoction fumigation hands.The treatment was for 3 weeks,before and after treatment(after treatment for 3 weeks after admission)and 3 months after discharge,the nailfold microcirculation was performed once,and the patient's finger distance and skin hardness score were recorded before and after treatment.The Graph Pad Prism 5 software was used to analyze the statistics and observe the changes in the data of each indicator.Result: 1)The score of nailfold microcirculation was significantly lower in the follow-up of the blood circulation and phlegm-staining group after 3 weeks of treatment and after 3 months of discharge.The score of nailfold microcirculation before treatment was 6.3±2.17,and the score of nailfold microcirculation was 4.42±1.53 after treatment,compared with P=0.0003 before treatment.The score of nailfold microcirculation was 4.61±1.45 after 12 weeks after treatment.The comparison P=0.0008;the P values of both groups were less than 0.05,which was statistically significant.2)The number of tube sputum after treatment at the end of treatment of Huoxuehuayu fumigation and washing group was 6.07±0.64/mm,compared with before treatment,P=0.0084,P<0.05,which was statistically significant.The number of tube defects after the end of treatment in the steam fumigation group was 5.70±0.79 pieces/mm,which was statistically significant compared with that before treatment(P=0.0287).It can be seen that the Huoxuehuayu fumigation group can not only increase the number of tube defects in the microcirculation of the nail,but also has more obvious effects than the steam fumigation group? 3)The number of deformed ducts in the Huoxuehuayu fumigation group was 0.53±0.78/mm after treatment,which was statistically significant compared with that before treatment.The number of malformations in the 12 th week after treatment was 0.57 ± 0.73 strips / mm,compared with before treatment P = 0.0078,statistically significant.It is indicated that the Huoxuehuayu fumigation can effectively reduce the number of deformed tube defects in the nailfold microcirculation in patients with systemic sclerosis.After the treatment of steam fumigation group,the number of deformed tube defects was 0.97±0.81/mm,which was statistically significant compared with that before treatment(P=0.0430).However,the effect of steam fumigation group was not obvious.4)After 3 weeks of treatment,the liquid integral was 0.83±0.46,compared with P=0.0004 before treatment,and the liquid integral was 1.04±0.68 at the 12 th week after treatment,compared with P=0.0107 before treatment.Both P values were statistically significant.5)The skin hardness score before treatment was 23.73±9.86,and the skin hardness score was 15.93±7.12 after 3 weeks of treatment;the skin hardness score of the steam fumigation group before treatment was 17.80±8.41,after 3 weeks of treatment The post-skin hardness score was 13.40±6.13.The P values before and after treatment were <0.05,which was statistically significant,and the Chinese medicine fumigation group P=0.0009,P<0.01,which was more statistically significant than the steam fumigation group P=0.0241.accurate.It shows that Chinese medicine fumigation can reduce skin hardness score more effectively than steam fumigation.Conclusion: The method of Huoxuehuayu Decoction fumigation can significantly improve the nailfold microcirculation abnormality in patients with systemic sclerosis complicated with Raynaud's phenomenon,and thus can improve the peripheral circulation of patients.Steam fumigation also improves the patient's nailfold microcirculation,but the curative effect is not as good as traditional Chinese medicine fumigation.This therapy may be effective for Raynaud's phenomenon and extremity lesions in SSc patients,which deserves further research.
