Objective:(1)Theoretical study on Jianpi Qinghua Fang(2)To observe the effect of Jianpi Qinghua Fang on improving renal function and pathological changes in rats with acute renal ischemia-reperfusion injury.(3)From the perspective of improving oxidative stress injury and inflammatory reaction,and looking for the preventive effect of Jianpi Qinghua Fang on acute renal ischemia-reperfusion injury.Provide a theoretical basis for clinical prevention and treatment.Methods:(1)Theoretical research: Through the understanding and theoretical research on kidney disease in traditional medicine,find and organize the theoretical basis of Jianpi Qinghua.(2)Experimental study: Rat model of acute renal ischemiareperfusion injury induced by bilateral renal artery ligation was established,and physiological saline,Jianpi Qinghua Fang and rosiglitazone hydrochloride were given for 1 week before modeling.After modeling,1 hour after modeling,6h after modeling,renal function,serum SOD,NO,NOS and hydroxyl radical inhibition ability of each group were determined in batches 24 hours after model establishment.Serum IL-1? was determined by ELISA.IL-10,TNF-?,Western blot analysis of renal tissue NF-?B expression levels,renal tissue HE staining,renal pathological changes in rats.Results:(1)Theoretical research: spleen deficiency and dampness heat is an important factor in the traditional Chinese medicine theory of acute kidney IRI.Jianpi Qinghua method is an effective method to prevent acute renal IRI.(2)Experimental study: Compared with the normal group,the BUN and Scr of the three groups were significantly increased(P<0.05),and the rosiglitazone hydrochloride group and the Jianpi Qinghua prescription group BUN were ischemia-reperfused for 1 hour.Scr was lower than that of the model group,and the Scr level was lower in the Jianpi Qinghua Fang group(P<0.05).The BUN and Scr of the rosiglitazone hydrochloride group and the Jianpi Qinghua Fang group were significantly lower than those in the model group(P<0.05).The BUN and Scr of the rosiglitazone hydrochloride group and the Jianpi Qinghua Fang group were significantly lower than those in the model group(P<0.05).The three groups of Jianpi Qinghua are better than rosiglitazone hydrochloride.Jianpi Qinghua Fang can effectively improve the pathological changes of renal tissue,and there is no significant difference between the group and rosiglitazone hydrochloride.Conclusion:(1)The acute renal IRI modeling method is easy to operate and successful by clamping the renal artery in the dorsal bilateral kidney.And acute renal IRI persisted and aggravated within 24 hours of reperfusion.(2)Prophylactic use of Jianpi Qinghua can significantly improve renal function in rats with acute kidney IRI,protect kidney function,and reduce renal function damage caused by acute renal IRI.(3)Prophylactic use of Jianpi Qinghua can significantly reduce renal damage in rats with acute kidney IRI,and can effectively improve pathological damage such as renal tubular injury.(4)The mechanism of Jianpi Qinghua Fang in protecting acute renal IRI may be related to enhancing the clearance of ROS and inhibiting the release of inflammatory factors NK-?B,IL-1? and TNF-?.(5)The pathological mechanism of acute renal IRI may be related to NF-?B/TNF-?/IL-1? signaling pathway,and Jianpi Qinghua can effectively inhibit this pathway and protect renal function.(6)Acute renal IRI may be in a state of damp heat,and Jianpi Qinghua may effectively protect kidney function by clearing away heat and dampness.
