Objective To compare dual-source CT's and routine enhanced CT's efficiency and reliability of scanning in cervical metastatic lymph nodes of oral squamous cell carcinoma,and to explore whether it is feasible and clinical value to use dual-source CT to examine cervical metastatic lymph nodes of oral squamous cell carcinoma in clinical practice,so as to improve the level of accurate surgical treatment.Materials and Methods The data of patients treated in the East Hospital Ward of Shandong Provincial Hospital Associated To Shandong University from April 2020 to September 2020 were collected,and all cases diagnosed with oral squamous cell carcinoma by pathological results obtained from preoperative biopsy were compiled.Among them,16 patients underwent skull base to neck root plain enhanced CT scan and 18 patients underwent skull base to neck root dual source CT scan.The metastatic lymph nodes were diagnosed according to the metastatic lymph node diagnostic criteria of ordinary enhanced CT and dual-source CT,and the postoperative pathological diagnostic results were taken as the gold standard.The statistical data were collected in a "group-group" manner,and the true positive number,false positive number,etc.S of the two examinations were calculated according to a four-compartment table.Make comparison and statistical analysis of the diagnostic test parameters of dual-source CT and normal enhancement CT for metastatic lymph nodes in the neck of OSCC.Results The true positive number,false positive number,true negative number,false negative number,sensitivity,diagnostic specificity,accuracy,positive predictive value,negative predictive value,false negative rate,false positive rate,positive likelihood ratio and negative likelihood ratio of routine enhanced CT were 10?4?18?6?62.5%?81.82%?73.68%?71.43%?75%?18.18%?37.5%?3.4375?0.4583,The data of dual-source CT were 9?3?35?6?81.82%?92.11%?89.80%?75,00%?94.59%?7.89%?18.18%?10.3636?0.1974,Kappa consistency test results showed that the coincidence rate between the diagnosis of dual-source CT and the gold standard of pathology was Po=0.898,Kappa=0.716(P<0.05),the Kappa value was greater than 0.7.The coincidence rate between the diagnosis of dual-source CT and the gold standard of pathology was Po=0.737,and the Kappa value=0.451,with moderate consistency(P<0.05).Conclusion1.Dual-source CT is a clinical adjunct method with high accuracy for the identification of OSCC cervical metastatic lymph nodes and reactive hyperplastic lymph nodes,and its results are highly consistent with the postoperative pathology.2.Compared with conventional enhanced CT,dual-source CT can diagnose neck lymph node metastasis of oral squamous cell carcinoma more accurately,and its data are superior to conventional enhanced CT by statistical calculation.3.Dual-source CT,as a new enhanced CT introduced in recent years,has high diagnostic efficacy and reliability for cervical metastatic lymph nodes of oral squamous cell carcinoma.It is feasible and promising to apply dual-source CT in clinical examination of cervical metastatic lymph nodes of oral squamous cell carcinoma.
