Application Of Frailty Assessment In The Management Of Elderly Patients With Heart Failure

Objective: While scale was used to screen the frailty of elderly patients with heart failure.To analyze the relationship between cardiac structure and function changes and frailty in elderly patients with heart failure.To follow-up and record the clinical endpoint events in elderly patients with heart failure and to study their relationship with frailty.Methods: A total of 100 elderly patients with heart failure hospitalized in the Geriatrics Department of Yi Jishan Hospital of Wannan Medical Collegel and Wuhu Second People's Hospital from August 2019 to June 2020 were collected.There were 49 males and 51 females with an average age of 76.68 and 8.87 years.1.While scale was used to screen 100 elderly patients with heart failure for frailty,According to the screening results,100 elderly patients with heart failure were divided into three groups: frailty group,profrailty group,and non-frailty group.2.Cardiac structure function and clinical endpoint events were recorded in the above three groups.Heart structure and function included EF – valued,LVEDD,6-minute walking test,BNP and heart function grade.The clinical endpoints included the number of unconventional readmissions,the number of out-of-hospital relapse,the number of falls,and the number of deaths.3.The data obtained in this study were uniformly sorted and numbered,and sp SS26.0software was used for analysis and processing.Test level A =0.05 was used.The measurement data were tested for normality,and the standard deviation of mean was used to describe the normality.The t-test was performed.Those that do not meet the normality test are described by median(Q25,Q75)and row rank sum test.The chi-square test of the counting data rows,if the applicable conditions are not met,the Fisher exact probability method is applied.Spearman correlation analysis was used to determine the P value,and P<0.05 was considered statistically significant.Results:1.A total of 100 elderly patients with coronary heart disease were included in this study.There were 49 males(49%)and 51 females(51%).The number of non-frailty patients was 20(20%),the number of frailty patients was 41(41%),and the number of frailty patients was 39(39%).47 patients(47%)had grade II cardiac function,48patients(48%)had grade III cardiac function,and 5 patients(5%)had less grade IV cardiac function.2 people died(2%).2.Age,EF,BNP,6 minute walking experiment,cardiac function grading and number of readmissions for heart failure may be an influence factor of weakness.The P values were 0.000,0.036,0.000,0.000,0.000,0.000(all P < 0.05).Gender,LVEDD(left ventricular end-diastolic diameter),and cardiovascular death had no statistically significant impact on fthenia,with P values of 0.900,0.742,and 0.206,respectively(all P>0.05).3.Bonferroni multiple comparisons were performed for age,EF,BNP,6-minute walk,cardiac function grading,and hospitalization for heart failure.The results showed that the older the age was,the higher the risk of frailty was,all P values <0.05,and the difference was statistically significant.A multiple comparison of BNP data of different frailty patients in the three groups showed that there was no difference between the pre-frailty group and the non-frailty group(P>0.05),and there was a difference between the frailty group and the non-frailty group(P<0.05).BNP value in the frailty group was higher than that in the non-frailty group(P<0.05).Multiple comparisons of data from the three groups of the six-minute walk experiment showed that the more severe the frailty,the worse the walking experiment results were(P<0.05).Multiple comparisons were conducted among the three NYHA groups,and the differences between the non-frailty group and the pre-frailty group were statistically significant(P<0.05).The number of patients with grade III cardiac function in the pre-frailty group was higher than that in the non-frailty group.The difference between the frailty group and the non-frailty group was statistically significant(P<0.05).The frailty group had more patients with grade III and IV cardiac function.The higher the frailty index was,the higher the grade of cardiac function was.Multiple comparisons were made between the three groups of heart failure readadmission.The difference between the pre-frailty group and the non-frailty group was not statistically significant(P>0.05).The difference between the non-frailty group and the frailty group was statistically significant(P<0.05),and the number of heart failure readmission times in the frailty group was more than that in the non-frailty group.Spearman rank correlation results showed that correlation coefficient R =-0.253,P=0.011<0.05,and frailty was negatively correlated with EF value.Conclusion: Frailty has a high detection rate in elderly patients with heart failure and increases with age.The grade of heart function,BNP and the number of readmission of heart failure in frailty patients were higher than those in non-frailty patients.The6-minute walking distance of frailty patients was shorter than that of non-frailty patients.Frailty is negatively correlated with EF.EF decreases as frailty index increases.It is suggested that attention should be paid to the management of frailty in elderly patients with heart failure.
