Background and Aim: Elderly patients with New-onset seizures are the first time with seizures.They need long-term medication after twice epileptic seizure,causing a huge economic burden on society.Previous studies have found that high uric acid(UA)is associated with post-stroke epilepsy may promote it.Seizures may also lead to high UA,but the relationship between high UA and seizures is not clear.This study explored the effect of high UA on New-onset epilepsy after stroke by observing changes in UA levels in patients with different etiologies of New-onset epilepsy and ordinary stroke patients.Methods: A retrospective collection of patients who were diagnosed with epilepsy for the first time in the Second Affiliated Hospital of Nanchang University from 2017 to 2020 and admitted to the hospital for 24 hours,patients who visited our hospital and were diagnosed with epilepsy many times,according to records when we first diagnosed epilepsy in our hospital Cases screen out patients who meet the conditions for the first visit.Based on the medical history and clinical symptoms,combined with auxiliary examinations such as electroencephalogram and craniocerebral magnetic resonance,the etiology of the patient will be clarified.A total of 526 stroke patients older than 60 years were randomly selected from our hospital emergency records.To classify all patients,in order to determine whether high UA is different in different causes of first epilepsy,the first epilepsy is divided into 1.stroke,2.trauma,3.encephalitis,4.tumor,and later to identify the seizures and high UA Whether it is related,177 patients are divided into 1.Stroke epilepsy,2.Non-stroke epilepsy,3.Ordinary stroke group,after the above three groups are grouped by gender to determine the effect of gender on UA level,and finally for the first time after clearing different types of stroke Is there any difference in UA level between epilepsy? The first epilepsy after stroke is divided into early-onset epilepsy group and late-onset epilepsy group,non-epileptic state group and epilepsy state group.Data analysis was performed using SPSS 22.0.Results: A total of 177 cases were collected,including 115 cases of first epilepsy and 62 stroke patients.The first epilepsy was divided into four groups.Serological indicators in the stroke group,trauma group,encephalitis group and tumor group were statistically analyzed.Only uric acid There were statistical differences,and the uric acid was analyzed separately.The four groups were analyzed by one-way ANOVA.The results showed that the uric acid level in the stroke group was significantly different from that in the trauma group,encephalitis group,and tumor group(P <0.001).All patients were divided into stroke epilepsy group,non-stroke epilepsy group and ordinary stroke group.The chi-square test was performed on the composition ratio of the number of people with high uric acid.The result showed that the composition ratio of the number of people with high uric acid in the stroke epilepsy group was significantly higher than that of the other two groups.The difference was statistically significant Significance(P<0.001).Univariate analysis of variance of uric acid levels among the three groups showed that the uric acid level in the stroke epilepsy group was significantly higher than that in the other two groups(P<0.001),indicating that seizures had no significant effect on uric acid levels.High UA is highly related to the first epilepsy after stroke.High UA can promote the first epilepsy after stroke.The patients were analyzed by gender and chi-square test was performed on the proportion of male and female population with high uric acid.The results showed that there was no significant difference in the proportion of male and female population with high uric acid(P=0.682).The chi-square test showed that the proportion of people with high uric acid after first stroke epilepsy was significantly higher than that of the other two groups of patients.The difference was statistically significant(P<0.001,P=0.006).A single factor chi-square test was performed between the early-onset epilepsy group and the late-onset epilepsy group,and the patients with ordinary stroke group.There was a significant difference between the three groups between the ordinary stroke group and the two groups(P<0.001).For the epilepsy status and non-epilepsy status groups,independent sample t test was performed according to gender between the two groups,and there was no significant difference in the level of uric acid between the epilepsy status and the non-epilepsy status between male and female stroke groups(P=0.363,P =0.929),whether the uric acid level in different causes of first-episode elderly men has diagnostic significance for the first-epidemic seizure after stroke is analyzed by ROC curve,and the area of AUC is obtained,AUC=0.923,P<0.001,and the men's uric acid is calculated using the Yoden index The critical value is 440.5umol/l,the sensitivity is 0.79,and the specificity is 0.99.Conclusion:(1)high uric acid is highly correlated with the first seizure after stroke.High uric acid can promote the first seizure after stroke,while the epileptic seizure will not lead to the increase of uric acid level.(2)gender has a certain influence on uric acid level.Uric acid value of > 440.5 living mol/L in elderly men after the first epileptic seizure is of certain diagnostic value for the diagnosis of the first epileptic seizure caused by stroke.
