Application Of Pentazocine Combined With Ropivacaine In Thoracic Paravertebral Nerve Block Perioperative Influence Of Lung Cancer Patients Undergoing Thoracotomy

Objective:To investigate the effect of thoracic paravertebral nerve block with pentazocine and ropivacaine on perioperative analgesia and stress response in patients with lung cancer undergoing thoracotomy.Methods:Sixty patients with lung cancer undergoing thoracotomy in our hospital were randomly divided into three groups:general anesthesia group?group A?,ropivacaine thoracic paravertebral nerve block+general anesthesia group?group B?,pentazocine combined with ropivacaine thoracic paravertebral nerve block+general anesthesia group?Group C?,20 cases in each group.The plasma levels of Norepinephrine?NE?,Epinephrine?E?and Cortisol?COR?were recorded at the time of admission?T0?,one hour after operation?T1?,postoperation?T2?,one hour postoperation?T3?,six hours postoperation?T4?,12 hours postoperation?T5?and 24 hours postoperation?T6?;the levels of T3-T6 were recorded visual analogue score?VAS?.The time of operation,the amount of remifentanil used during operation,the times of PCA self-control administration within 24 hours after operation were recorded;The incidence of adverse reactions such as nausea and vomiting,dizziness,respiratory depression,hypertension,tachycardia and arrhythmia were counted.Results:There was no significant difference in general condition and operation time?P>0.05?.The dosage of remifentanil in group C and B was significantly lower than that in group A,and the dosage of remifentanil in group C was significantly lower than that in group B?P<0.05?;the times of PCA self-control administration in group B and C within 24 hours after operation were significantly lower than that in group A,and the times of PCA self-control administration in group C within 24 hours after operation were significantly lower than that in group B?P<0.05?,the VAS score of group B and group C within 24 hours after operation was significantly lower than that of group A?P<0.05?,and the VAS score of group C within 24 hours after operation was significantly lower than that of group B?P<0.05?;the NE and E of group B and group C at T2T6 were significantly lower than those of group A?P<0.05?,the NE of group C at T4T6 was significantly lower than that of group B?P<0.05?,the E of group C at T3T6 was significantly lower than that of group B?P<0.05?;the Cor of group B and group C at T1T6 was significantly lower than that of group A,The Cor of group C was significantly lower than that of group B at T4T6?P<0.05?.The incidence of adverse reactions in group C was lower than that in group A and B at 24hours after operation.Conclusion:The application of patazoxin combined with ropivacaine in thoracic paravertebral nerve block can provide good perioperative analgesia for lung cancer patients undergoing thoracotomy,reduce their perioperative stress response and reduce the incidence of postoperative adverse reactions.
