| Objective:To investigate the correlation between the expression of hepatocyte nuclear factor 1-alpha(HNF1α)in liver tissues of patients with chronic hepatitis b(CHB)and the grade of liver inflammation and the stage of liver fibrosis and its clinical significance.In order to help assess the disease,to find an effective way to intervene or block the formation and progression of liver fibrosis.Methods:Sixty-four patients with CHB who were diagnosed and treated in our hospital from September 2011 to October 2018 were selected,All the patients underwent liver biopsy under the guidance of B ultrasound.Pathological results of liver biopsy were collected,including routine HE staining and Masson staining,and inflammatory grading(G)and fibrosis staging(S)were completed.The paraffin blocks of liver tissues were collected and sectioned,adopting immunohistochemical staining to detect the expression of HNF1αin liver tissues,and analyzing the correlation between the expression intensity of HNF1αand inflammation grading and fibrosis staging.Mantel-Haenszel chi-square test was used for the bidirectional ordered grouping data.Kruskal-Wallis test was used for the difference analysis of multi-group grade data,and Spearman rank correlation test was used for the correlation analysis.Results:1.Pathological results of liver biopsyHE staining and Masson staining indicated that the liver tissues of patients with CHB presented different levels of inflammatory necrosis and fibrosis,including inflammation grading:10 cases of G0,18 cases of G1,19 cases of G2,13 cases of G3and 4 cases of G4.Fibrosis staging:10 cases of S0,15 cases of S1,23 cases of S2,11cases of S3,and 5 cases of S4.2.Immunohistochemical resultsHNF1αwas widely distributed in the liver cytoplasm,and positive expression was also found in the liver nucleus.The expression intensity of HNF1αwas different in liver tissues with different levels of inflammation and fibrosis.The chi-square test of Mantel-Haenszel showed that there was a linear trend between the expression intensity of HNF1αand the grade of liver inflammation(χ2MH=40.70,P<0.05).Kruskal-Wallis test indicated that there were differences in the expression intensity of HNF1αin liver tissues with different grades of inflammation(H=42.09,P<0.05).The chi-square test of Mantel-Haenszel showed that there was a linear trend between the expression intensity of HNF1αand the stage of liver fibrosis(χ2MH=31.95,P<0.05).Kruskal-Wallis test indicated that there were differences in the expression intensity of HNF1αin liver tissues with different stages of fibrosis(H=32.82,P<0.05).Spearman rank rank correlation test showed a significant negative correlation between the expression intensity of HNF1αand liver inflammation grading(rs=-0.815,P<0.05)and liver fibrosis staging(rs=-0.713,P<0.05).Conclutions:The expression level of HNF1αis closely related to the occurrence and development of liver lesions,and is expected to be a sensitive index to evaluate the level of liver inflammation and fibrosis in patients with CHB.HNF1αis likely to be an inhibitor of liver inflammation and liver fibrosis,and its down-regulation may be an important factor in the formation and progression of liver fibrosis. |
