Objective:Lacrimal duct plug is a common treatment method for dry eye syndrome.There are many materials for lacrimal duct plug in clinic,but there are also many side effects.Corneal stromal lenticule are derived from human avascular tissue,and there is no research on the use of SMILE-derived corneal stromal lenticule from human.As an implant material for the study of dry eye,the corneal matrix derived from SMILE was made into a lacrimal duct plug and implanted in the lacrimal duct of the rabbit to observe the effect on dry eye.Methods:Forty1.8 kg-2.0 kg healthy male New Zealand white rabbits were selected,8 rabbits were randomly selected as the normal control group,and the remaining 32 were used 0.1% benzalkonium chloride(BAC)to induce the dry eye model(right eye,4 times / day,continuous)14 days).Tear secretion volume(SIT),break up time(BUT),corneal fluorescein staining(FL)scoring data to determine the success of modeling,14 days after modeling,screen white rabbits that meet the dry eye criteria,randomly divided into two group: Group A: Lacrimal plug treatment group,B: dry eye model group,normal control group as group C,the tear secretion(SIT),break up time(BUT),corneal fluorescein staining(FL)and corneal confocal microscopy were measured at 2 weeks,4 weeks,and 6 weeks after intervention.At 6 week,eye samples were collected for histological examination,confocal microscope examination,transmission electron microscope(TEM)examination and Weston blot examination.Results:Compared with pre intervention,the SIT and BUT of the A and B groups were significantly reduced(P <0.05)and the FL score was significantly increased(P <0.05).At 4 and 6 weeks after the lacrimal embolization,The SIT and BUT of the group A increased significantly(P <0.05),and the FL score decreased significantly(P <0.05).The SIT and BUT in group B were significantly lower than those in groups A and C(P <0.05),and the FL score was significantly increased(P <0.05).After 6 weeks of implantation,HE staining results showed that the number of corneal epithelial cells in group A tended to be normal,The surface is smooth,the corneal epithelium in group B is significantly thicker than that in group A and C(P <0.05),the cell arrangement is disordered,PAS staining results show that the number of conjunctival epithelial layers in group A is basically normal,and the number of goblet cells is the same as in group C compared with no significant difference(P> 0.05),the conjunctival goblet cells in group B decreased significantly,and the difference was statistically significant compared with groups A and C(P <0.05).Transmission electron microscopy showed that the two groups in groups A and C corneal epithelial microvilli are arranged in a regular and finger-like manner.On the contrary,a group of disordered,shorter and fewer microvilli were observed in group B.Confocal microscopy results showed that the number of inflammatory cells in group A decreased significantly compared with group B(P <0.05).Western Blot test showed that the expression levels of corneal NF-κB and p-NF-κB in group A were significantly reduced.Conclusions:Implantation of SMILE-derived corneal stromal lenticule lacrimal plug has a certain effect on the treatment of dry eye syndrome in rabbits.
