Objective To investigate the effect of blood inflow angle on intracranial aneurysm ruptured hemorrhage and to predict the risk of unruptured intracranial aneurysm ruptured hemorrhage.Methods A total of 208 patients with intracranial aneurysm hospitalized in the department of neurosurgery of the second affiliated hospital of nanchang university from June,2016 to December,2010 were collected.The included patients were divided into two groups,among which 139 patients had ruptured intracranial aneurysm and 69 patients had unruptured intracranial vein tumor.Get intracranial aneurysms by DSA examination,and 3D vascular images of responsibility artery,determined by measuring blood flow into the point of view,and through the statistical analysis of the ruptured and unruptured intracranial aneurysms in the angle size whether there is a difference,and determine whether the different angle of blood inflow will affect the rupture of intracranial aneurysm.Results Among the 208 patients with intracranial aneurysm included in the measurement,the average angle of blood flow inflow was 107.0±24.3° in the ruptured hemorrhage group and 98.1±23.0° in the non-ruptured group.Compared with the two groups,the angle of blood flow inflow in the ruptured hemorrhage group was greater than that in the non-ruptured group.The independent sample test of the mean value of the blood inflow angle found(P=0.012)that there was a significant difference in the angle between the ruptured bleeding group and the unruptured group,which was statistically significant.Univariate logistic regression analysis was used to study the effect of blood inflow angle on ruptured hemorrhage.The OR value was 1.015(95%CI: 1.003-1.028),and the probability of aneurysm ruptured hemorrhage increased by 1.5% for every 1 degree increase of blood inflow Angle.Among thesepatients,14 were mirrored aneurysms.Independent sample test was conducted on the mean value of the blood inflow angle of mirrored aneurysms,and the difference of the blood inflow angle between left and right aneurysms(P=0.020)showed that there was a difference between the two.Among the reexamination patients,9 cases of intracranial aneurysms were found to have changes in morphology,and there was a difference between the anterior and posterior mean blood flow inflow angles(P=0.039).There is statistical significance between the two.Conclusion By analyzing the difference of blood inflow angle between the ruptured group and the unruptured group,it is found that the blood inflow angle is correlated with intracranial aneurysm ruptured hemorrhage.The greater the angle of inflow,the greater the risk of rupture.The difference of blood inflow angle between the two sides of the mirror aneurysm and the changes before and after the blood inflow angle of the reviewed patients verified that the blood inflow angle was related to intracranial aneurysm rupture hemorrhage.The conclusion shows that there is a certain correlation between the increase of inflow angle and the change of probability of rupture hemorrhage.In the assessment of the risk of ruptured intracranial aneurysm hemorrhage,the angle of intracranial aneurysm blood inflow can be appropriately referred to.
