Objective:To investigate the effect of desflurane and propofol anesthesia on early postoper ative cognitive function in elderly patients.Methods:The patients?grade ASAIIIII?who underwent radical resection of gastrointe stinal tumors were randomly divided into desflurane group?group D?and propofol group?group P?.Group D maintained anesthesia with remifentanil 0.10.2?g/?kg.min?continuously pumped and simultaneously inhaled desflurane 26%after induction in the same manner;Group P maintained anesthesia with remifentanil 0.1 and0.2?g/?kg.min?and propofol 46?kg.h?.the recovery time and extubation time of the two groups after anesthesia were recorded;the concentration of S100?protein was detected at two points before and after anesthesia;the scores of 1 day?T0?before operation,1 day?T1?after operation,3 days?T2?,5 days?T3?and 7 days?T4?were evaluated and recorded using the MMSE scale.Result:1.There was no statistical difference in general preoperative data between two groups?P>0.05?;there was no significant difference in operation time and anesthesia time between the two groups?P>0.05?;there was statistical difference in extubation time between the two groups?P<0.05?,and the extubation time the Group D was significantly shorter than that of the Group P.2.Compared with T0,the MMSE of the two time points T1 and T2 in the two groups were decreased,and the difference was statistically significant?P<0.05?;but there was no statistical difference in the score of each time point between the two groups.3.Compared with preoperative,the S100?protein concentration was increased1 d after operation,and the MMSE score was decreased,the difference was statistically significant?P<0.05?;but there was no difference in protein concentration between the two groups?P>0.05?.4.The incidence of early cognitive dysfunction in group D and group P was 11.8%and 17.6%,respectively.There was no significant difference in the incidence of POCD between the two groups?P>0.05?;Conclusion:1.Compared with propofol,for elderly patients undergoing gastrointestinal tumor surgery,desflurane produced similar POCD rates.2.Compared with propofol,for elderly patients undergoing gastrointestinal tumor surgery,Desflurane inhalation anesthesia and propofol have similar hemodynamic stability and can be safely used in clinical practice.
