PURPOSE:To explore the efficacy of nasal endoscopic and external dacryocystorhinostomy(DCR)guided by CT dacryocystography for patients with chronic dacryocystitis.METHODS:60 patients(60 eyes)with chronic dacryocystitis from January 2019 to September 2019 were collected.All cases in the study were divided into the study group and the control group,30 cases respectively.The study group was treated with nasal endoscopic DCR(internal DCR)and the control group was treated with external DCR.Then all patients were given preoperative routine examination,lacrimal passage irrigation and CT dacryocystography and three-dimensional reconstruction.All patients were treated with Absorbable Gelatin Sponge stuffing in the DCR,and were treated with Triamcinolone Acetonide Nasal Spray spray for three months.The patients were successively observed the symptoms of lacrimal discharge,lacrimal passage flushing,anastomotic healing and major complications in 1,3 and 6 months after surgery.SPSS 25.0 software was used to analyze the data,P < 0.05 was regarded as the difference of statistical significance.RESULTS:(1)The total effective rate in the first month after operation: 96.7% in the study group and 100% in the control group,there was no statistical significance between the two groups(P > 0.05);the total effective rate in the third month after operation:96.7% in the study group and 86.7% in the control group,there was no statistical significance(P > 0.05);the total effective rate in the sixth month after operation:93.3% in the study group and 90% in the control group,there was no statistical significance(P > 0.05).(2)The pain score of the study group was lower than that of the control group(P < 0.05).The pain score of three months after operation was lower than that of one week among two groups(P < 0.05).(3)The operation time of the study group was shorter than that of the control group(P < 0.05).The length ofhospital stay in the study group was shorter than that in the control group(P < 0.05).(4)There was no significant difference between the two groups in the incidence of bleeding in one week and in the incidence of anastomotic granulation and anastomotic stenosis in six months(P >0.05).CONCLUSION:(1)Both of the two methods have high curative effect.(2)With the guidance of CT dacryocystography,nasal endoscopic DCR has no skin incision,which is more intuitive and minimally invasive during the operation,and has shorter operation time,hospitalization time and lighter pain after the operation and more advantages.
