Object:The STOP-BANG questionnaire was used widely for detecting patients with obstructive sleep apnea.However,the assessment of the applicability of the questionnaire for screening for sleep apnea syndrome in Chinese hypertensive populations is lacking.The body mass index(BMI)cut-off value in the original STOP-BANG questionnaire was 35 kg/m2;the neck circumference(NC)cut-off value was40 cm,but this cut-off point value needs to be verified in this population.Our goal was to assess the diagnostic efficacy of the STOP-BANG questionnaire for this population and to establish a more appropriate BMI and neckline cutoff for this population in the STOP-BANG problem.Methods:Collect patients who underwent polysomnography(PSG)and completed the STOP-BANG questionnaire in the Hypertension Department of the People's Hospital of the Autonomous Region from2016 to 2018 to evaluate the accuracy of the STOP-BANG questionnaire in predicting severe OSA(AHI?30 events/h)and Evaluation of the suitability of the modified STOP-BANG questionnaire for this population.Results:A total of 284 patients completed the experiment,When the BMI cut-off value is set to 28 kg/m ~2 and the neck circumference cut-off point is set to 42 cm,the area under the ROC curve of the STOP-BANG scale is the largest(0.81,p<0.001).The sensitivity and specificity of the original ST-BANG questionnaire for predicting severe OSA in hypertensive patients were(80.58%and 60.69%),respectively,and the area under the ROC curve was(0.755,p<0.001).In the scale,when the neck circumference is unchanged and the BMI cutoff value is changed from 35 to 28,the AUC of the scale is changed from 0.755(0.70-0.80)to 0.784(0.73-0.83),and the difference is statistically significant(P<0.01).When the BMI was unchanged and the neck circumference changed from 40 to 42 in the scale,the AUC of the scale changed from 0.755(0.70-0.80)to 0.785(0.73-0.83),and the difference was statistically significant(P<0.01).When the BMI and neck circumference cutoff values were changed to 28 kg/m~2 and 42cm respectively,the area under the curve was 0.81(0.76-0.85),the difference was statistically significant compared with the original questionnaire,only changing the cut-off value of the neck circumference and only changing the cut-off value of BMI(P<0.01).the sensitivity of the modified questionnaire was 87.77%(81.1-92.7),the specificity was 62.07%(53.6-70.0),and the Youden index was 0.50.Conclusion:The STOP-BANG questionnaire has a high sensitivity and moderate predictive value for screening this population.We recommend that the questionnaire use a BMI>28 kg/m~2,a neck circumference>42cm,and a STOP-BANG score?3 is the cutoff value.Identifying patients with high-risk OSA in patients with hypertension.
