Ulnar Nerve Anterior Or Not For Distal Humerus Fracture Meta Analysis Of The Effect Of Operation

Objective: In view of the patients with distal humeral fracture without ulnar nerve disorder before operation,the effects of two different treatment methods,anterior ulnar nerve and in situ release,were compared by meta analysis,which provided some reference for clinicians in future work.Methods: Formulated after retrieval keywords in both Chinese and English by the Chinese hownet(CNKI),VIP(VIP),ten thousand party platform,Chinese biomedical database(CBM),Pubmed,the Cochrane library,Embase,and Web of science at home and abroad medical literature retrieval database,all documents retrieved by inputting Endnote software according to include and exclude standard carries on the preliminary screening.After that,the selected literatures were evaluated according to their different research methods by corresponding standards,and the literatures that finally met the standards were included in the study.In this study,the operative time,intraoperative blood loss,fracture healing time,incision infection rate,excellent and good rate of postoperative elbow joint function and incidence of postoperative ulnar neuritis were taken as the analysis indexes,and the relevant data were extracted.The Review Manager5.3 software was used to perform Meta analysis and draw sensa and funnel plots.Results: The comparison of different treatment methods of ulnar nerve between the two groups only showed statistical difference in operation time(P< 0.00001),but there was no statistical significance in terms of intraoperative blood loss(P=0.31),fracture healing time(P=0.26),incision infection rate(P=0.90),excellent and good rate of postoperative elbow function(P=0.21),and postoperative ulnar neuritis incidence(P=0.39).Conclusion: According to the results of the meta-analysis,in patients with preoperative without ulnar nerve injury symptoms,open reduction and internal fixation in the distal humerus fractures,the ulnar nerve front with in situ release on the surgical curative effect and no significant difference,and in view of the in situ release relatively short operation time,simple operation,reduce the risk of iatrogenic injury,so usually don't recommend routine pre ulnar nerve.However,the surgeon still needs to carefully select a more appropriate individual program according to the actual condition of theaffected limb and its own operation level during the operation.
