Objective To analyze the clinical and pathological characteristics of colorectal polyps in children,and to explore the clinical value of painless colonoscopy,to provide a reference value for clinicians in the diagnosis and treatment of colorectal polyps in children.Methods The clinical data of 590 children undergoing painless colonoscopy from July 2017 to December 2019 in Jiangxi Children’s Hospital were retrospectively analyzed.According to the age of the child,it is divided into infants and young children(< 3 years old),preschool age(3 to 6 years old),school-age(7 to11 years old)and adolescence(12 to 16 years old),and the clinical and pathological characteristics are compared.Results In this study,a total of 590 children underwent 608 painless colonoscopy,428 males and 162 females,male: female = 2.64:1,the median age was5.63 years(8 months to 16 years),medium The course of disease was 6 months(1day to 10 years),including 77 cases of infants and young children,295 cases of preschool age,161 cases of school-age and 57 cases of puberty.The symptoms of590 children were 511(86.61%)with blood in the stool,37(6.27%)with abdominal pain,11(1.86%)with diarrhea,6(1.02%)with constipation and 25(4.24%)with other symptoms.Intestinal lesions were detected in 385 cases(65.25%),including302 cases(78.44%)with colorectal polyps,47 cases(12.21%)with chronic colitis,8cases(2.08%)with Crohn’s disease,and ulcerative colon There were 5 cases of inflammation(1.30%)and 23 cases of other lower digestive tract diseases(5.97%).The distribution of colorectal polyps,chronic colitis,inflammatory bowel disease and other digestive tract diseases in children of different ages was statistically significant(P <0.01);among them,colorectal polyps accounted for a higher proportion of preschool age,chronic colon Inflammatory bowel disease accounts for a high proportion of pre-school age and school-age,and inflammatory bowel disease accounts for a high proportion of school-age and adolescence.The insertion rate of the cecum was 82.40%(501/608).Among the 302 polyps,single polyps accountedfor 88.41% and multiple polyps 11.59%.Polyps are mainly located in the rectum and sigmoid colon,accounting for 81.46%.Pathological results indicated that juvenile polyps accounted for 98.53%,melasma polyps syndrome accounted for 1.10% and inflammatory polyps accounted for 0.37%.There were no anesthesia-related accidents(such as cardiac arrest and respiratory arrest)in 590 children.After the examination,2 patients had post-anesthesia tremor;302 patients with colorectal polyps except for 9 patients with spontaneous polyps,and 293 patients with colon Microscopic high-frequency electrical snare coagulation resection,in which 5 cases had intraoperative bleeding,2 cases had postoperative bleeding,and 2 cases had postoperative bowel perforation.Conclusions Painless colonoscopy is a safe and effective method for diagnosing children’s lower gastrointestinal diseases.Children of different ages have different distribution of lower gastrointestinal diseases;children’s colorectal polyps are mainly distributed in the rectum and sigmoid colon,and the pathological types are mainly young polyps.,Single occurrence is more common;endoscopic removal of colorectal polyps is a safe and effective method.
