| This study explores the epidemiological characteristics and pathogenic spectrum data of patients with diarrhea syndrome in the sentinel hospital of Panjin City from 2018 to 2019,and explores the antibiotic resistance of clinically used antibiotics of major pathogenic microorganisms.The construction of public health incident prevention and control system provides objective and quantitative modern medical indicators,and optimizes the conditions for rapid detection of bacteria by time-of-flight mass spectrometry according to the biological characteristics of bacteria,aiming to improve the ability of bacteria detection.The main contents of the research include:1.Analysis of clinical patient information.The patients who were treated at the sentinel hospital in Panjin City and diagnosed with diarrhea syndrome were taken as the research object.Through the investigation of basic information of the case,laboratory pathogenic microorganism detection and drug resistance of common clinical antibiotics The combined method of detection collects basic information such as the patient’s age,sex,diarrhea days and clinical symptoms,analyzes the characteristics of diarrhea syndrome cases in Panjin City,and strengthens the construction of a public health event prevention and control system inPanjin City.2.Pathogen spectrum and drug resistance The pathogen spectrum of patients with diarrhea syndrome was detected;the antibiotic resistance of the main pathogenic microorganisms of diarrhea syndrome was analyzed,and clinical medication can be guided according to the results of drug sensitivity tests.3.Optimize the conditions for rapid detection of bacteria by time-of-flight mass spectrometry Select 10 strains of Vibrio parahaemolyticus for the optimization experiment of rapid detection of bacteria by time-of-flight mass spectrometry: 37 ℃ in TCBS medium,general nutrition plate,gentamicin agar plate After 16 hours of culture,10 strains of Vibrio parahaemolyticus were analyzed on the time-of-flight mass spectrum to match the mean value of Score Value ± standard deviation;10 strains of Vibrio parahaemolyticus were cultured on TCBS medium at 37 ℃ for 8 h,12 h,16 h,20 At h and 24 h,the analysis is to match the mean value of the Score Value ± standard deviation on the time-of-flight mass spectrometry,and the optimized conditions for the detection of bacteria using the time-of-flight mass spectrometry are obtained from the obtained mean value of the Score Value ± standard deviation.The statistical analysis of the stool samples sent by the sentinel hospital shows that the patients have the largest number of patients on theday of diarrhea.The main clinical symptoms are diarrhea,abdominal pain,and vomiting.The color of the stool is mostly watery stools and loose stools.The onset time is mostly July,August and September.After cultivating and identifying bacteria in 725 patients,4 types of bacteria,namely,diarrhoeal Escherichia coli,non-typhoidal salmonella,Vibrio parahaemolyticus,and Aeromonas hydrophila,were detected,accounting for 5% and 0.55% of the total,9.79%,0.28%;after PCR virus detection of 500 patients,only one norovirus was detected,accounting for 6.80% of the total.From the above results,it can be seen that the main pathogenic microorganism of the diarrhea syndrome in Panjin City is Vibrio parahaemolyticus.The analysis of Vibrio parahaemolyticus by region,age,and gender shows that Xinglongtai District has the highest proportion of patients,accounting for 60.56% of the total positive number;There is a certain difference in the number of men and women(male: female = 1: 1.29);the infection rate of Vibrio parahaemolyticus in the 18-45 age group is the highest,accounting for 67.61 of the total positive rate.The drug sensitivity test of 16 drugs was conducted on 71 cases of Vibrio parahaemolyticus.The test results showed that Vibrio parahaemolyticus was sensitive and sensitive to 10 drugs such as cefotetan,norfloxacin,chloramphenicol,and gentamicin.The rate was100%;100% resistance to methamphetamine,97.18% resistance toampicillin,88.73% resistance to sulfamethoxazole;and cefuroxime axetil,cefuroxime,and cefazolin were Moderately resistant.The optimized experimental results of rapid detection of bacteria by time-of-flight mass spectrometry show that under different conditions but with the same culture time,10 strains of Vibrio parahaemolyticus were cultured on TCBS medium,ordinary nutrition plates,and gentamicin agar plates.The results of the time-of-flight mass spectrometry and the matching Score Value of the MS library are the mean ± standard deviation of 2.453 ±0.030,2.237 ± 0.041,and 2.325 ± 0.055 respectively;under the same culture medium but different culture time,10 strains of Vibrio parahaemolyticus Incubation on TCBS plates for 8 h,12 h,16 h,20 h,and 24 h,the mean ± standard deviations of the results of the time-of-flight mass spectrometry and the matching Score Value of the mass spectral library are 2.103 ± 0.012,2.215 ± 0.024,2.453 ±0.030,2.339 ± 0.061,2.205 ± 0.057.Analysis of the above results shows that the main incidence of diarrhea syndrome in 2018-2019 is July,August and September,and the age is concentrated between 18-45 years old.The main pathogenic microorganisms are Vibrio parahaemolyticus and Norovirus;parahaemolytic arc The highest detection rate was obtained when the bacteria were cultured in TCBS medium at 37 ℃ for16 h.This experiment successfully explored the epidemiologicalcharacteristics and pathogen spectrum data of patients with diarrhea syndrome in Panjin City,as well as the antibiotic resistance of commonly used clinical antibiotics of major pathogenic microorganisms;and according to the biological characteristics of bacteria,the conditions for rapid detection of bacteria by time-of-flight mass spectrometry The optimization will lay the foundation for revealing the epidemiological
