The Application Value Of Anti-CarP Antibody For Rheumatoid Arthritis

Objective: To investigate the expression level and application value of anti-CarP antibody in rheumatoid arthritis.Methods: From December 2018 to June 2019,there were 158 patients with RA and 59 patients without RA,including 30 patients with OA,7 patients with SLE,11 patients with AS,11 patients with UCTD and 42 healthy controls.The demographic data,laboratory results and serum samples of the patients were collected,and the concentration of anti-CarP antibody in the serum of each subject was determined by ELISA method for statistical analysis.Results:1.The concentration of anti-CarP antibody in RA group was higher than that in non-RA group and healthy control group.There was no significant difference in the concentration of anti-CarP antibody between non-RA group and healthy control group(P<0.05).The concentration of anti-CarP antibody in non-RA group was higher than that in non-RA group and healthy control group.2.ROC curve analysis showed that the sensitivity of anti-CarP antibody in the diagnosis of RA was 58.2%,the specificity was 93.1%,and the area under the curve was 0.81.Compared with the specific index of RA diagnosis,the sensitivity of RF in the diagnosis of RA was 63.9%,the specificity was 85.1%,and the area under the curve was 0.74.The sensitivity of anti-CCP antibody in the diagnosis of RA was 71.5%,the specificity was 96.0%,and the area under the curve was 0.84.When anti-CarP antibody,anti-CCP antibody and RF were detected jointly,the area under the curve was 0.92,the sensitivity was 82.3%,and the specificity was 96.5%.3.Univariate logisitic regression analysis showed that age,CRP,ESR,RF,GPI,anti-CCP antibody and anti-CarP antibody were risk factors for RA.Multivariate logisitic regression analysis showed that anti-CCP antibody and anti-CarP antibody were independent risk factors for RA.4.Spearman correlation analysis showed that anti-CarP antibody was positively correlated with CRP,but not with SJC,TJC,ESR,DAS28,CDAI and SDAI.5.Further analysis showed that the positive rate of anti-CarP antibody in serum-negative RA group was 44.4%.6.The concentration of anti-CarP antibody in RA bone erosion group was higher than that in RA non-bone erosion group,and there was significant difference between the two groups(P<0.05).Conclusion: 1.Anti-CarP antibody is an effective serological marker for RA diagnosis.2.The combined detection of RF,anti-CCP antibody and anti-CarP antibody can improve its diagnostic value,and anti-CarP antibody can be used as an effective assistant diagnostic tool for serum negative RA.3.The high level of anti-CarP antibody in the serum of patients with RA may indicate an increased risk of bone erosion,which should be intervened early,but further cohort studies are needed for follow-up observation.
