Objective: To investigate the value of serum transferrin(Tf)in the early diagnosis of sepsis.At the same time,the levels of procalcitonin(PCT)and c-reactive protein(CRP)were compared with thecurrent clinical indicators for early infection diagnosis of sepsis.To evaluate the value of serum transferrin level in the early identification of sepsis.Methods: From January 2018 to June 2019,192 patients with suspected sepsis in emergency department,EICU and ICU were selected.Within 24 hours of onset,without any treatment,blood samples were collected to detect the levels of PCT,CRP and Tf.According to the sepsis3.0 diagnostic criteria,192 patients were divided into ordinary infection group,early sepsis group and septic shock group.The septic shock group was not included in the study criteria because septic shock requires adequate fluid resuscitation.The values of PCT,CRP and Tf in the early sepsis group and the common infection group were compared between the groups,and the differences between the groups were analyzed.Then,the rank correlation coefficients of Tf,PCT and CRP were compared.Since all the indexes in the three groups were not normally distributed,the detection data were expressed as(P25,P75).ROC curves were also drawn for PCT,CRP and Tf indicators to compare the sensitivity,specificity,accuracy and cut-off values of the above three inflammatory indicators for the diagnosis of early sepsis.Spss21.0software was used for statistical analysis of the final data,and P < 0.05 was statistically significant.Results:1.There was no significant difference in age and gender between the early sepsis group and the common infection group(P >,0.05).2.PCT10.84(9.13,12.68),CRP83.4(71.38,99.63)in the early sepsis group were all higher than that in the common infection group(PCT2.27(1.15,3.74)and CRP51.30(33.90,83.00),while Tf1.4205(1.2192,1.7015)in the early sepsis group was lower than that in the common infection group(1.6360,1.5540,1.8100),with significant differences(P < 0.05).3.Rank sum test of PCT,CRP and Tf in early sepsis showed negative correlation with PCT and CRP.The correlation coefficient between Tf and PCT rank was-0.353,and that between Tf and CRP rank was-0.389,indicating significant correlation(P <0.05).4.The ROC curve of early sepsis was drawn with the common infection group as the control group.The areas under the curve of PCT,CRP and Tf were 0.962,0.762 and0.738,respectively.The sensitivity,specificity and accuracy of Tf were 67.90%,68.10%and 68.04% respectively,which were less predictive than PCT and CRP.Conclusion: Tf level can be used as an inflammatory indicator for the early diagnosis of sepsis,providing a new biomarker for the early identification of sepsis.
