Purpose:Using 3.0 T resting state functional magnetic resonance imaging(rs-fMRI)and diffusion tensor imaging(DTI),in bilateral thalamus interested area,through the resting state functional connectivity(FC),based on the analysis of the voxel(VBA)method and nuclear magnetic resonance image processing software was used to study nerve myelitis spectrum disorders(NMOSD)function of the thalamus,the change of microstructure and volume as well as the correlation of cognitive function change,for cognitive dysfunction in patients with NMOSD imaging and theoretical foundation for possible pathogenesis.Methods:Twenty NMOSD patients(NMOSD group)and 12 healthy controls(HC group)were selected.All subjects underwent a Montreal cognitive assessment(MoCA)score and an extended disability scale(EDSS)score 1 day prior to the MRI.Subjects were scanned with BOLD-EPI,3DT1W1 and DTI by 3.0t magnetic resonance scanner.FC analysis was used to calculate the functional connection coefficients between the thalamus and the whole brain on the left and right sides,and the differences in functional connection between the thalamus and the whole brain between the two groups were compared.VBA method was used to compare the changes of anisotropic fraction(FA)and mean diffusivity(MD)in the whole brain,left and right thalamus regions of the experimental group and the control group.Using three-dimensional volume workstation Aquariusws software to obtain the experimental group and the control group’s volume of left and right lateral thalamus and carries on the statistical analysis between the two groups,using SPSS25.0 software on two groups of subjects’ cognitive dysfunction incidence of chi-square test,the general situation and Montreal cognitive assessment(MoCA)score in two independent sample t-test,NMOSD MoCA score,EDSS score and group differences between brain regions FC value and FA value,MD,the left and right after the standardization in the hypothalamus,where the volume between the two correlation analysis.Results:(1)Compared with HC group,the incidence of cognitive function in NMOSD group increased significantly,with statistically significant difference.(2)Compared with HC group,the left side of the thalamus and the left anterior cingulate,back to the last time,right amount in the left frontal,central,on the right side of the top right cingulate gyrus area function enhanced connection,and on the left side of the central front cover back,the left side of the island of inferior frontal gyrus,inferior frontal gyrus on the left side of the triangle department,limbic lobe,vice gyri,corpus callosum,the right side of the triangle in the inferior frontal gyrus,right frontal,back on the right side Luo Landi island lid,right before the central back to weakened function connection,on the right side of the supplementary motor area.(3)Compared with the HC group,on the right side of the thalamus and the left prefrontal,left anterior cingulate,right edge lobe,right insula,right angular gyrus,right cerebellum function enhanced connection,and the left,the left side of the tongue back,fusiform edge of leaf,parahippocampal gyrus,left the,on the left side of the triangle inferior frontal gyrus,precentral gyrus of right frontal lobe,right before the central back,back,right in the right frontal dorsolateral frontal gyrus,right orbitofrontal amount last time,the right orbit inferior frontal gyrus,supplementary motor area on the right side weakened function connection.(4)Compared with HC group,FA value in right thalamus of NMOSD group increased,while MD value in NMOSD group did not change significantly.(5)Compared with HC group,the bilateral thalamic volume of NMOSD patients decreased significantly,and the difference was statistically significant.(6)There was a positive correlation between left and right thalamic volume and MoCA score after standardization in NMOSD patients.Conclusions:(1)The incidence of cognitive dysfunction was significantly higher in the NMOSD group than in the HC group.(2)In patients with NMOSD,the functional connection between the bilateral thalamus and the whole brain is enhanced and weakened,among which the functional connection between the right thalamus and the right frontal lobe is related to cognition,while the functional connection between the left thalamus and the whole brain is related to cognition and the degree of disability.(3)Patients with NMOSD have abnormal thalamic microstructure and volume atrophy.(4)There was a correlation between bilateral thalamic volume and cognitive function in NMOSD patients.
