Objective:The accuracy of high field intensity MRI(3.0 T)in diagnosing supraspinatus tendon injury was studied by using arthroscopy as the gold standard,and the relationship between supraspinatus muscle injury and shoulder joint movement was discussed.Materials and methods:A retrospective analysis was performed on 46 patients with suspected rotator cuff injury in the second hospital of jilin university between March 2017 and December 2019,and the 46 patients underwent MRI and arthroscopic examination of shoulder joint with high field strength in our hospital successively.There were 19 males and 27 females.Male: female ? 1:1.42,mean age(54.54±13.30)years.Using arthroscopy as the gold standard,the significance of the association between arthroscopy and MRI was evaluated by chi-square test,and the consistency between arthroscopy and MRI for partial and complete supraspinatus tear was analyzed by Kappa test.All patients were double-blind scanned by two imaging physicians who had more than 5 years' experience in imaging diagnosis of the supraspinatus muscle,and further analyzed and calculated the accuracy of MRI diagnosis of the supraspinatus tendon injury and injury type.By summarizing and analyzing the patient's medical history,MRI and arthroscopic results,the relationship between supraspinatus tendon injury and shoulder joint motion was further discussed.Results:The sensitivity of MRI in diagnosing supraspinatus tendon injury was 96.15%,the specificity was 75.00%,the positive predictive value was 83.33%,the negative predictive value was 93.75%,the missed diagnosis rate(i.e.,false negative rate)was 3.85%,the misdiagnosis rate(i.e.,false positive rate)was 25%,the Youden index was 0.71,and the coincidence rate was 86.96%,,MRI and arthroscopy in the diagnosis of presence of supraspinatus damage was statistically significant(P < 0.05).The sensitivity of MRI in diagnosing full-thickness tear was 85.00%,the sensitivity of partial tear was 83.33%,the Youden index was 0.68,and the coincidence rate was 84.62%.MRI diagnosed partial and complete supraspinatus tear with P values less than 0.05,Kappa value was 0.612.Therefore,MRI examination was highly consistent with arthroscopy.Supraspinatus muscle injury is more common in the right shoulder or dominant arm,and degeneration is the basic factor,which is related to more times of movement and more lifting movement.The mechanism is mostly subacromial impingement syndrome.Conclusion:MRI has a high sensitivity and specificity in the diagnosis of supraspinatus tendon injury,which is mostly found in the right shoulder or dominant arm,which is related to the number and direction of movement,and is mostly caused by subacromial impingement syndrome.The size and range of supraspinatus tear,the involvement degree of adjacent structure involvement,muscle atrophy,tendon contraction and other information can be observed in MRI images,which h is of great significance for clinical treatment decision-making.
