Objective:The effects of general anesthesia combined with saphenous nerve block and general anesthesia alone on the quality of recovery of patients during postoperative anesthesia care unit(PACU)were compared and analyzed,and the intraoperative medication and hemodynamic fluctuations were compared between the two groups at the same time.In order to explore a method of anesthesia that shortens the length of stay in the PACU,reduces complications after anesthesia,and allows patients to recover more steadily.Methods:After screening of inclusion and exclusion criteria,52 patients with unilateral total knee arthroplasty were selected,including 26 patients in group S(saphenous nerve block and general anesthesia)and 26 patients in group C(general anesthesia).The basic and intraoperative blood pressure,heart rate,consumption of cisatracurium and opioids during anesthesia,equivalent conversion of opioids to morphine equivalent,PACU stay,extubation time,and the incidence of complications of hypoxemia,agitation,nausea and vomiting(PONV),respiratory depression were reviewed.The t-test,chi-square test and other statistical methods were used to analyze the differences between the two groups.Results:There was no significant difference in basic data between the two groups: age,gender,ASA grade,BMI score,operation time,presence or absence of hypertension,heart disease,diabetes,basal mean arterial pressure and basal heart rate(P> 0.05).The mean arterial pressure in group S was higher than that in group C at 60 minutes and90 minutes after anesthesia,and the difference was statistically significant,P < 0.05.The heart rate in group S was significantly lower than that in group C at 10 minutes afteranesthesia,and the difference was statistically significant,P< 0.05.There were significant differences in heart rate as well as mean arterial pressure in group S(P < 0.01).There were significant differences in heart rate as well as mean arterial pressure in group C(P <0.01).Overall,the stability of mean arterial pressure and heart rate in group S was greater than that in group C,and the difference was statistically significant(P < 0.05).The incidence of complications within the PACU was: nausea and vomiting(2/52,3.8%);agitation(1/52,1.9%);hypoxemia(4/52,7.6%);and the incidence of respiratory depression was zero.There was no significant difference in individual postoperative complications between the two groups(P > 0.05).However,the total complications were less in group S than in group C,and the difference was statistically significant,P< 0.05.Compared with group C,the PACU stay and anesthesia time of group S were significantly shorter,and the difference was statistically significant(P < 0.05);however,there was no difference in extubation time between the two groups,P > 0.05.Compared with group C,the use of cisatracurium and opioids was significantly reduced in group S,and the difference was statistically significant,P < 0.05.Conclusions:Saphenous nerve block is used for general anesthesia of patients undergoing unilateral total knee arthroplasty,which can make the intraoperative blood pressure and heart rate more stable and improve the quality of recovery.
