Methods a retrospective study was conducted on 410 patients who were first admitted to the neurological intensive care unit of our hospital from January,2017 to October,2019.Inquire clinical data,including general demographic indicators(age,gender),baseline levels of factors that may affect the occurrence of SAP(records of admission cases and results of relevant laboratory tests),and categorize and record the patient’s consciousness status,medical operation,length of stay and outcome inStroke-associated pneumonia(SAP)is an infectious pulmonary parenchymal(alveolar wall)inflammation in the acute phase in a stroke patient who has no prior pulmonary infection.Studies have shown that SAP is an independent risk factor for death in stroke patients and can significantly increase hospital stay and medical costs.In recent years,with the attention and research of many scholars at home and abroad,SAP has clearly defined diagnostic criteria,its pathophysiological mechanism is complex,risk factors are numerous,and there is no consensus on treatment and preventive measures.But while saving lives and improving cure rates,the neurological intensive care unit NICU also has a high concentration of susceptible people and risk factors,making it a high risk site for infections.Prevention,diagnosis and treatment of SAP in early NICU patients with vasoplasty are increasingly becoming one of the key factors in patient outcomes.Therefore,the purpose of this study was to explore the incidence,risk factors and severity of SAP in patients treated with early NICU angioplasty,so as to improve the understanding of SAP among neurointensists and interventional physicians and improve the relevant diagnosis and treatment measures.the ward.SPSS22.0 statistical software was used for data analysis,X2 test was used for counting data,t test and rank sum test were used for measurement data,and relevant risk factors affecting the occurrence of SAP were screened out.By fitting the multivariate unconditional Logistic regression model and adjusting the related confounding factors,the independent risk factors for SAP were found.Results a total of 410 patients were enrolled,including 309 males(75.4%)and 101 females(24.6%).The average age was 60.24+0.961 years.There were 241 cases of SAP,and the total infection rate was 58.8%,of which 181 were males,accounting for 75.1%,and 60 were females,accounting for 24.9%.There were 78 cases treated with thrombolysis,230 cases with mechanical thrombolysis,95 cases with stent implantation,and 7 cases with balloon dilatation.There were significant differences in the incidence of SAP among patients with different early angioplasty treatments(P<0.05).Pneumonia with pneumonia group between age,early endovascular treatment,and the history of high blood pressure,stroke history,onset vomiting,disturbance of consciousness before and after onset,preoperative and postoperative NHISS score,NIHSS score difference,postoperative Glasgow grading,tracheal intubation and tracheal intubation time,tracheotomy and differences statistically significant(P < 0.05).Postoperative disturbance of consciousness,time of endotracheal intubation,NIHSS score at onset and stent implantation may be independent risk factors for SAP in patients treated with early vascular opening in the neurological intensive care unit,among which postoperative disturbance of consciousness,time of endotracheal intubation and relative risk of preoperative NIHSS score are relatively high.Conclusion the incidence of SAP in patients with early NICU angioplasty was 58.8%.Postoperative appear disturbance of consciousness,endotracheal intubation time,preoperative NIHSS score,early opening of treatment for stent implantation may be for this kind of independent risk factors in patients with SAP,the history of high blood pressure,in the course of vomiting,onset and postoperative disturbance of consciousness,endotracheal intubation and tracheotomy may be an associated risk factors of pulmonary infection score.
