Objective: To analyze the clinical effect of soft tissue balance procedure with modified rotational Scarf osteotomy for the correction of moderate-severe hallux valgus.Methods: According to the inclusive and exclusive criteria,some cases were included by retrospectively evaluating the clinical data of hallux valgus patients from January 2016 to January 2017.Hallux valgus angle(HVA),Tibial sesamoid position(TSP),Intermetatarsal angle(IMA),Distal metatarsal articular angle(DMAA),American Orthopaedic Foot and Ankle Society-Hallux-Metatarsophalangeal-lnterphalangeal(AOFAS-Hallux-MTP-IP)scale,Visual analogue scale(VAS)at preoperation and last follow-up and complications during follow-up period were investigated.Results: All the 46 cases were followed up for 20-26 months(mean,23.4 months).HVA,IMA,DMAA and TSP were all significantly improved(P<0.05),a higher AOFAS score and a lower VAS score were achieved at last follow-up(P<0.05).2 patients had transfer metatarsalgia and 1 patient had poor healing on the surface of the incision site during follow-up period.Conclusion: Soft tissue balance procedure with modified rotational Scarf osteotomy can restore the relationship between sesamoids and metatarsal bone and distal metatarsal articular angle through translating laterally and rotating the plantar osteotomy,so it is worth popularizing application in treating moderate-severe hallux valgus because of good postoperative satisfaction,lower recurrence rate and less complications of the patients in our study.
