OBJECTIVES: To evaluate the prognosis of acute myocardial infarction(AMI)by using the Stress Hyperglycemia Ratio(SHR).Method:644 patients diagnosed as AMI were enrolled.Patients' medical history information and biochemical indicators(with special emphasis on immediate blood glucose and glycosylated hemoglobin)were collected and stress hyperglycemia ratio(SHR)of each subjects were calculated.According to the occurrence of in-hospital and 2-year major adverse cardiovascular,cerebrovascular events(MACCEs)and the situation of diabetes patients were divided into several groups.Indicators above were evaluated to predict the prognosis of patients with AMI and compared the difference of blood sugar and SHR in predicting the outcome of AMI patients.Results: The proportion of patients with AMI complicated with diabetes mellitus,SHR(P = 0.003)and the ratio of SHR(P = 0.004)were significantly higher in patients with long-term events.Among the AMI patients with diabetes mellitus,SHR in the hospital event group was significantly higher than that in the events-free group(P < 0.001).Immediate blood glucose(P < 0.001)and SHR(P < 0.001)were higher in the long-term event group than in the non-event group.No matter with or without diabetes mellitus,all-cause mortality was the highest in the group with the highest SHR value(P = 0.044,P = 0.008).AMI patients with and without DM were further divided into high SHR group and low SHR group.High SHR could better predict in hospital MACCEs rate in AMI patients with DM as well as 2-year MACCEs rate without DM.Conclusion: Stress hyperglycemia is an important indicator for poor prognosis in patients with AMI during long-term follow-up.Compared with immediate blood glucose at admission,the value of stress hyperglycemia ratio is more valuable in predicting hospital adverse events and long-term MACCEs in patients with AMI,and the advantage is not different whether or not diabetes is complicated.
