Chinese clinics have been facing the problem of outpatient overcrowding for long time.The relatively concentrated visit time of patients form an obvious arrival peak period,during which time the outpatient hall and all the consultation areas are overcrowded,leading patient waiting time greatly exceeds the medical service time.To ease this situation,large hospitals in China nowadays usually adopt a mixed queuing policy of online appointment with on-site queuing,in hope that would shunt the arrival of some online appointment patient,eventually alleviating the prominent contradiction between medical resources and demand in peak period.However,this practice has extended the waiting time of walk-in patients,increased the difference of waiting time between two types of patients,and has caused serious dissatisfaction of walk-in patients.Therefore,the administration of hospital should pay attention to the comprehensive satisfaction degree based on waiting time of both appointment patient and on-site patient.In this paper,a real-time scheduling strategy for outpatient patients' appointments based on prospect theory is studied and designed.Firstly,by consulting the research results at home and abroad,this paper combs the research situation of outpatient appointment and patient dispatching strategy.Then,taking the endocrine department of Shanghai Sixth People's Hospital as the main research object,through field investigation and statistical analysis,this paper makes an in-depth study on the appointment situation of the general outpatient department in recent years,summarizes the reasons that may lead to the patient's dissatisfaction,and combines the prospect theory formula,The utility value function describing the satisfaction of individual patients is constructed,and the formula for calculating the comprehensive satisfaction of patients is put forward.By designing different patient scheduling strategies,constructing various optimization objective functions,using the method of simulation optimization,this paper evaluates the performance of the comprehensive satisfaction of each scheduling strategy under different optimization objective functions,and further explores the influence of various patient psychological reference modes on the comprehensive satisfaction degree.Finally,this study uses genetic algorithm to optimize the allocation of reservation number source in each time period,and tries to realize the further improvement of patients ' comprehensive satisfaction without increasing the total resource consumption of the system.Through the comparison test of various optimization objective functions and the sensitivity analysis test of key parameters,it is proved that the proposed optimization strategy of this study has been significantly improved compared with the original strategy,and has a certain application prospect.
