Objective: To observe the changes of hemoglobin concentration and erythrocytes count in 3 days after transfusion of intraoperative blood salvaged erythrocytes which were washed with multiple electrolyte injection or normal saline. Methods: A total of 46 patients who under went orthopedic surgery and required blood recovery were randomly divided into normal saline group(group NS,n=24,the red blood cells salvaged were washed with normal saline)and multiple electrolyte injection group(group MEI,n=22,the red blood cells salvaged were washed with multiple electrolyte injection). The blood(2m L/person)was taken from venous of patients before surgery, immediately after surgery and 1-3 days after surgery respectively.Hb and RBC were detected and operation time,in vitro time of red blood cells salvaged, washing time of red blood cells salvaged, the volume of intraoperative blood loss, net red blood cells salvaged input, urine output, infusion volume, postoperative drainage volume were recorded in both groups. Results: 1.There were no differences in preoperative general conditions, operation time, vitro time of red blood cells salvaged, washing time of red blood cells salvaged, the volume of intraoperative blood loss, net red blood cells salvaged input, urine output, infusion volume, postoperative drainage volume between two groups(P<0.05).2.The changes of hemoglobin concentration.Intra-group comparison showed that it was decreased significantly that of concentration of hemoglobin at the immediately postoperative than that of preoperatively(P<0.05)and no statistical difference between 1 day postoperative and immediately postoperative, but it was decreased significantly on 2 day and 3 day postoperative and both lower than that of 1 day postoperative(P<0.05)in group NS.The immediately postoperative concentration of hemoglobin was significantly decreased compared with that before operation(P<0.05)and that on 1 day postoperative was significantly increased compared with that of immediately postoperation, but it was significantly reduced on 2 day and 3 day postoperative and both lower than that of 1 day postoperative in group MEI(P<0.05).Inter-group comparison showed that the concentration of hemoglobin was increased in the MEI group than that in the NS group on 1day postoperative(P<0.05)and no difference was noted between the two groups on other time points.3.The changes of red blood cells count. The intra-group comparison of red blood cells count of group NS was declined conspicuously at the immediately after surgery compared with that before surgery(P<0.05)and that on the 1st after surgery was similar to the immediately after surgery, however,it was declined conspicuously on the 2nd and 3rd after surgery and both fewer than the 1st after surgery(P<0.05).The immediately after surgery of red blood cells count of group MEI was remarkablely declined compared with that before operation(P<0.05)and that of red blood cells count on the 1st after surgery remarkablely increased than of immediately after surgery(P<0.05),while it was declined remarkablely on the 2nd and 3rd after surgery compared with that on the 1st after surgery. The between-group comparison of red blood cells count in group MEI was higher than that in group NS on the 1st day after surgery(P<0.05),the parameters at other time between the two groups were similar without any remarkable differences.4.Correlation analysis revealed a negative correlation between the volume of intraoperative blood recovery and the trough postoperative hemoglobin concentration in the two groups. Conclusion: IBS washed by multiple electrolyte injection could delay the decrease of hemoglobin contents and erythrocytes count after the transfusion of intraoperative blood salvaged erythrocytes.
