Objective:In order to clarify the influence of obesity on the changes of blood lipid metabolism,inflammatory factors and prognosis in rats after burns.Burn models and obesity models were established through animal experiments,and then theinflammatory factors;blood lipid metabolism;and skin growth status were also determined to analyze the risk of obesity on the prognosis of burns.The basicresearch data of the interactive effects of obesity and burn can provide reference for the treatment and nursing of special population(obese burn patients)and improve the prognosis.Methods:1.After obese model were successfully built,according to body weight,SD rats were divided into normal weight group,over-weight group and obesity groups(60 rats in each group).Each group was randomly divided into control group,?°burn group and?°burn group(20 rats in each subgroup),the burn group constructed10cm~2 wounds of?°burn and?°burn on the back,and monitored their body weight,photographed to record wound changes on days of 1st,3rd,7th,and 14th after burns.Rats'blood and skin samples were also collected.2.Full-automatic biochemical analyzer was used to detect TC,TG,LDL-C,HDL-C expression in serum on days 1st,3rd,7th,and 14th.3.Elisa test was used to detect the expression of IL-6 and IL-10 in serum.4.RT-PCR test was used to detect of the relative expression of IL-6,IL-8 and IL-10genes in burn wounds.5.Immunohistochemistry test was used to detect TGF-?1 and VEGF protein.6.Observe the skin pathological slide of HE staining to observe the skin growthstatus,the skin wound healing histopathology score was used to evaluate the wound healing status.7.Correlation analysis of healing rate with inflammation-related indicators,skin growth-related factors,and influencing factors of wound repair scores.Result:1.On the 1st day after the burn,the TG of the burn group was higher than that of the control group(P<0.05).On the third day after burn,the TG and HDL-C of burn in normal-weight group were higher than those of the control group,and theLDL-C levels of the burn group were lower than those of the control group(P<0.05).On the 7th and 14th days after the burn,the TG of the burn in over-weight and obese groups was higher than that in the control group,and in the normal-weight group,the HDL-C of burn group was lower than that of the control group(P<0.05).2.Elisa test showed that.on the 1st and 3th day after burn,IL-6 was high in obese group after?°burn than in the control group?°burn.On the 3rd day of burn,the expression of IL-10 protein in obese group after?°burn and?°burn was higher than that in control group(P<0.05).Severe burns could promote the expression of IL-6 in obesity group and normal-weight group.The change of IL-6 and IL-10 protein expression in degree of burn and obesity were not significantly different on the day after 7th,14th.3.RT-PCR test of burn wound tissue genes showed that after burns,the IL6 and IL-8genes in the obese group were higher than those in the normal group,and IL6 and IL-8 genes in over-weight group were higher in the normal-weight group(P<0.05).Severe burns tend to increase the IL6,IL-8,and IL-10 genes in the obese group.On the 3rd day of burn,the levels of obesity group and over-weight group of IL-10 gene were higher than those of normal-weight group(P<0.05).On the 7th day of burn,The level of IL-10 gene in the obese group is lower than that of over-weight group and normal group.On the 14th day after the burn,IL-10 gene in normal group was higher than that in obese group and over-weight group.In normal-weight group and over-weight group,the expression of IL-10 gene decreased with the increase ofdegree of burn(P<0.05).4.Immunohistochemistry test showed that in the obese groups and over-weight group,TGF-?1 protein in?°burn and?°burn burns was lower than that in the controlgroup(P<0.05).In normal-weight group,over-weight group,and obese groups,the expression of VEGF in?°burn was lower than that in the control group(P<0.05).On the day of 3rd,7th,and 14th days after the burn,as the degree of burn increased,the TGF-?1 protein and VEGF protein in the skin wounds of the normal-weight group,over-weight group,and obese groups all showed a downward trend.And in?°burn,the expression level of TGF-?1 protein was obesity group<over-weight group<normal-weight group.Severe burns had a tendency to reduce the expression level of TGF-?1 protein in obesity group(P<0.05).5.Skin wound healing histopathology score result showed that after burn,the skin wound healing histopathology score of?°burn was lower than those of?°burngroup(P<0.05).Moreover,the effects of the degree of obesity on skin wound healing histopathology score showed that the skin wound healing histopathology score of the obese group were lower than those of normal-weight group on the 7th and 14th days after the burn(P<0.05).6.Analysis factors affecting the wound healing rate and skin wound healing showed that IL-6 and IL-8 were negatively correlated with the healing rate after burns(r=-0.185,P<0.05;r=-0.252,P<0.05).There was a positive correlation between TGF-?1 and VEGF and the healing rate after burns(r=0.520,P<0.05;r=0.420,P<0.05).The analysis of influential factors of wound healing pathology score suggested thatIL-10 is a favorable factor skin wound healing histopathology score(RR=1.003,P<0.05).While IL-6 and obesity were risk factors for skin wound healinghistopathology score(RR=0.976,P<0.05;RR=0.720,P<0.05).Conclusion:1.After burns,TG levels in rats increased,TC,HDL-C,and LDL-C levels decreased.On the 3rd day of burn,LDL-C in the obesity group was higher than that of over-weight group and normal group.Obesity combined with burns had nosignificant effect on TG,TC,and HDL-C in rats in the(1,3,7,14)days.2.Obesity affects the levels of IL-6,IL-8 in burn rats and has a mutually reinforcing effect,while obesity decreases IL-10 level of burned rats.3.Obesity reduces expression of TGF-?1 protein in severely burned rats.IL-8 isnegatively correlated with the wound healing rate after burns.IL-6 and obesity factors are risk factors for skin wound healing histopathology score in burn rats,while IL-10is a favorable factor for skin wound healing histopathology score in burn rats in the3th,7th,14th days.Obesity can delay the healing time of the wound and make the prognosis of the burn worse.
