Objective:Peripheral white blood cell count(WBC)to explore the early stages of pregnancy,neutrophils/lymphocytes(NLR),and hepatic function index alanine aminotransferase(ALT),aspartate aminotransferase(AST)and the risk of gestational diabetes mellitus(GDM)occurred in the correlation,analysis of the predictive value of GDM,in order to GDM do early found early prevention,improve the outcome.Methods:A retrospective analysis was made of 500 pregnant women who underwent regular labor examination and gave birth with complete medical records in the second hospital of jilin university.According to OGTT results,84 cases were divided into case group(GDM group)and control group(normal glucose tolerance group,NGT group)and 174 cases.WBC and ALT levels were subdivided into groups,and the relative risk OR and 95% confidence interval CI of GDM were calculated.The relationship between WBC,NLR,ALT and AST in peripheral blood in early pregnancy and GDM,as well as the predictive value of GDM,were discussed through data analysis and the ideal cutoff value was obtained.Results:(1)comparison of WBC,NLR,ALT and AST in the early gestational period between the GDM group and the NGT group: the mean WBC,NLR,NLR,and AST in the GDM group was(9.01±1.58)×109/L,the mean NLR was 3.84±1.11,the mean ALT was(29.31±7.71)IU/L,and the mean AST was(20.56±9.57)IU/L.The average WBC of the NGT group was(7.35±1.04)×109/L,the average NLR was 3.71±1.09,the average ALT was(18.38±6.45)IU/L,and the average AST was(20.03±7.56)IU/L.(2)regression analysis was carried out on the correlation between the levels of WBC,NLR,ALT and AST in early pregnancy and the risk of GDM.The results showed that the OR level of serum WBC in early pregnancy was 4.066,and the 95%CI was 2.560-6.460,P < 0.05.The OR level of NLR in early pregnancy was 1.583,and the 95%CI was 0.946-2.663,P=0.079.The ALT OR level in early pregnancy was 1.080,and the 95%CI was 1.035-1.127,P=0.024.The OR level of AST in early pregnancy was 1.074,and the 95%CI was 0.973-1.203,P=0.126.(3)a subdivision comparison of WBC and ALT between the GDM group and the NGT group showed that the blood WBC level in the GDM group was increased,and the incidence of WBC≥8×109/L was increased,indicating that the risk of increased WBC(≥8×109/L)in the early pregnancy of GDM patients was increased,that is,the possibility of increased WBC was increased.In the GDM group,ALT levels increased and the incidence of ≥20IU/L increased,indicating that patients with GDM had an increased risk of ALT elevation(≥20IU/L)in early pregnancy,that is,the possibility of ALT elevation increased.(4)on the WBC < 8 x 109 / L,ALT < 20 iu/L as the control group,single factor analysis of different WBC,ALT levels and the risk of GDM occurred in the relationship,with the WBC,ALT elevations,GDM risk,risk of GDM in WBCc group,WBCb 45 times and 6 times the WBCa group respectively(P < 0.01),the ALTc group,ALTb were ALTa risk for GDM group 24 times and 4 times(P < 0.01).(5)relevant indicators were taken into the analysis of the subjects’ working curve,and it was found that the areas under the ROC curve were 0.782,0.700 and 0.826,respectively,with WBC,ALT and the combination of the two as the analysis indicators in early pregnancy.It indicates that both of them have better predictive value for the occurrence of GDM,and the value of the combined prediction of GDM is greater than that of the single prediction.(6)correlation analysis between early pregnancy WBC count and ALT level showed that there was a significant positive correlation between early pregnancy WBC count and ALT level,r=0.5,P<0,05.The elevation of white blood cells in the early pregnancy of GDM patients is often accompanied by the elevation of ALT,a liver function indicator.Conclusions:(1)early pregnancy in GDM patients is often accompanied by increases in WBC,NLR,ALT and AST,which are more sensitive than NLR and AST in predicting the correlation of the occurrence of GDM.(2)there was a positive correlation between the levels of WBC and ALT and the occurrence of GDM in early pregnancy.(3)the value of peripheral blood WBC and ALT combined in predicting GDM was greater than that of single prediction.(4)there is a correlation between WBC and ALT levels.Elevated WBC is often accompanied by elevated ALT,so the combined monitoring of the two should be strengthened in early pregnancy. |
