Background and objective:Stroke gain a lot of attention as the second leading cause of death in the world,and the leading cause of death in nation.Large hemispheric infarction?LHI?occurs as a consequence of large size of the ischemic lesion,involving2/3 of the area of the malignant middle cerebral artery territory.It may includes the anterior and/or posterior territory.This disease,which is associated with significant disability and mortality,is one of the most important causes of in-hospital mortality in stroke.As the mortality is high,the analysis of the clinical characteristics may be helpful in the management of LHI.The stroke mortality rate is higher in the old.The explanation of this phenomenon is not certainly clear.It is assumed that patients of different ages may have different causes of death.Clinical data were collected to explore some questions:what is the difference of clinical course between the young and the old?what is the influence of age on causes of in-hospital mortality?Method:Clinical data of patients,who were admitted to Department of Neurology of Jilin University 1st Hospital from June 1,2017 to December 31,2019,and met the inclusion criteria,were retrospectively collected.There were 49patients.All patients has radiographs to be confirmed to had new LHI.Patients were divided into two groups according to their age:<65 vs.?65 years.The medical history?presentation?medical examination?therapies?complications and causes of in-hospital mortality were recorded and then,statistically analysed with SPSS20.0 statistical software.Results:Compared with the younger age group,the older patients have some characteristics:the duration of hospital stay?P<0.001?was longer,coronary disease?P=0.025??atrial fibrillation?P=0.014?was higher,serum albumin?P=0.016?was lower.Compared with people in old group,more people in young group died of herniation,while fewer died of other organs disease?P=0.011?.Conclusion:Compared with younger died patients,the older ones may be more likely to be women.They also have higher rates of coronary disease,atrial fibrillation,and albumin level may be lower.Aging is related to a lower proportion of death of hernation,and longer to stay in hospital.After patients admitted to the hospital,more hypertonic dehydration for people over 65 years of age may mean nothing,while for the younger patients,longer life time.
