Objective: To investigate if the chlamydia pneumoniae(Cpn)bacteremia exists,and clarify the relationship between the Cpn bacteremia and clinical manifestation,when it exists using real-time real-time quantitative polymerase chain reaction method abbreviated to real-time PCR.To analysis the clinical charateristics of Cpn pneumonia,so as to provide references for the early diagnosis and treatment of children with Cpn pneumonia.Methods: From December 2017 to May 2019,65 children admitted to the first pediatric respiratory department of the First Hospital of Jilin University were chose as the experimental group,who were clearly diagnosed with Cpn pneumonia and met the inclusion criteria.37 children who were performed physical examination in our hospital in the same period were randomly chose as the control group.Serum and mononuclear cells were detected by real-time PCR.The positive rates of chlamydia were compared between the two groups.If the experimental group was positive for chlamydia,the clinical manifestations(age difference,antipyretic time,hospitalization time and complications)of the positive group and the negative group were compared.Retrospective analysis was conducted on the clinical data such as the clinical manifestations,auxiliary examinations,treatment and prognosis of these 65 patients.Results: 1.Serum samples of 65 children in the experimental group were detected by real-time PCR,and there was only 1 case with positive results of mononuclear cell extract,with a positive rate of 1.54%,and no positive results of serum detection.Serum and mononuclear cell extracts from the control group showed no positive results.2.The positive rates of serum samples and mononuclear cell samples were statistically analyzed,and the results showed no statistical significance.Cpn positive rate in the experimental group was not statistically significant compared with that in the control group.3.Among the 65 cases,all of them underwent cough,mostly irritative dry cough,52 cases(80.0%)had fever and 19 cases(29.2%)had wheeze,7 cases(10.8%)manifested pleural effusion,3 cases manifested diarrhea(4.6%),1 case(1.5%)manifested sinusitis,1 case(1.5%)manifested secretory otitis media,and 1 case(1.5%)manifested adenoid hypertrophy.Myocardial enzyme profile creatine kinase isoenzyme(ck-mb)increased in 13 cases(20.0%),alanine aminotransferase(ALT)increased in 3 cases(4.7%),erythrocyte sedimentation rate was detected in 32 cases and accelerated in 11 cases(34.3%).The main manifestations of chest X-ray examination were Hilar shadow thickening,patchy or flocculent shadow,accounting for 39.5% each.Conclusions: 1.The positive rate of Cpn specific fragments detected in the blood samples of children with Cpn pneumonia is relatively low,the occurance rate of Cpn bacteremia in children may be low.2.Cpn pneumonia can be characterized by cough,fever,wheezing,and multiple extrapulmonary manifestations.The prognosis is well after anti-infection treatment with azithromycin and symptomatic support treatment.3.Cpn pneumonia also accounts for a significant proportion of cases under 5 years of age,requiring hospitalization due to severe symptoms.
